基于MVPA和VBM的功能性消化不良患者大脑异常研究

【摘要】:功能性消化不良作为一种常见的胃肠病严重影响了患者的生活质量并且为患者带来巨大的医疗费用。近年来,神经成像技术,特别是磁共振成像技术和正电子发射断层技术,已经广泛地应用于功能性消化不良患者大脑异常活动的探究。许多研究表明,脑肠轴的功能紊乱是引起功能性消化不良发病的重要机制。然而,这些研究大部分使用基于实验的手段,很少有研究对患者静息态下大脑活动进行研究。因此,本论文主要通过磁共振成像技术研究功能性消化不良患者在静息状态下的大脑结构和功能的异常。 首先,,我们采用多体素模式分析对功能性消化不良患者和正常对照组在静息态下大脑的局部一致性进行研究。利用主成分分析对局部一致性数据进行降维,降维后的数据用以训练一个线性支持向量机分类器,我们可以得到组间的判别模式。然后再对该判别模式进行置换检验,得到具有显著组间差异的脑区,包括:前额叶皮层、眶额叶皮层、运动辅助区、颞极、脑岛、扣带回前部/中部、丘脑、海马/旁海马以及小脑。此外,部分脑区的局部一致性异常与功能性消化不良症状(病重和病程)相关。这些结果有助于人们进一步了解功能性消化不良的神经机制,并且表明多体素模式分析方法和功能磁共振成像对功能性消化不良的病理机制进行探究具有可行性。 其次,我们利用基于体素的形态测量学和基于种子点的全脑结构协方差分析探究功能性消化不良患者纹状体灰质变化及其与全脑的结构协方差模式。结果发现,双侧壳核和右侧尾状核灰质增加。与正常对照组相比,功能性消化不良患者与纹状体具有异常的结构协方差模式的脑区有:丘脑、杏仁核、海马/旁海马、舌状回和小脑。此外,我们还发现患者灰质异常的纹状体区域的灰质平均值与病程正相关。总之,这些研究结果表明功能性消化不良患者纹状体结构及其与全脑的结构协方差存在异常,并且为揭示功能性消化不良发病机制提供了进一步的支持。
[Abstract]:As a common gastrointestinal disease, functional dyspepsia has seriously affected the quality of life of patients and brought huge medical costs to patients. In recent years, neuroimaging technology, especially magnetic resonance imaging and positron emission tomography, It has been widely used to explore abnormal brain activity in patients with functional dyspepsia. Many studies have shown that dysfunction of the brain intestinal axis is an important mechanism for the pathogenesis of functional dyspepsia. Most of these studies use experiment-based methods, and few studies look at brain activity in patients at rest. Magnetic resonance imaging (MRI) is used to study the abnormal brain structure and function in patients with functional dyspepsia. First of all, we used multivoxel model analysis to study the local consistency of brain in patients with functional dyspepsia and normal controls under resting state. The principal component analysis (PCA) was used to reduce the dimension of local consistency data. The dimensionally reduced data are used to train a linear support vector machine classifier, and we can get the discriminant pattern between groups. Orbital frontal cortex, motor auxiliary area, temporal pole, cerebral island, anterior / central cingulate gyrus, thalamus, hippocampus / para-hippocampus and cerebellum. Local consistency abnormalities in some brain regions are associated with symptoms of functional dyspepsia (severity and course of disease). These results help to further understand the neural mechanism of functional dyspepsia. The results also indicate that multivoxel mode analysis and functional magnetic resonance imaging are feasible for exploring the pathological mechanism of functional dyspepsia. Secondly, we used morphometry based on voxel and covariance analysis of whole brain structure based on seed points to explore the striatal gray matter changes and their structural covariance patterns with the whole brain in patients with functional dyspepsia. The gray matter of bilateral putamen nucleus and right caudate nucleus increased. Compared with the normal control group, the functional dyspepsia patients and striatum had abnormal structural covariance pattern: thalamus, amygdala, hippocampus / para-hippocampus. In addition, we found a positive correlation between the average of gray matter in the gray matter abnormal striatal area and the course of disease. These results suggest that the striatum structure and its structural covariance with the whole brain are abnormal in patients with functional dyspepsia and provide further support for revealing the pathogenesis of functional dyspepsia.
