CT小肠造影对克罗恩病活动度的诊断价值

【摘要】：目的:探讨CT小肠造影(CTE)在评估克罗恩病活动度方面的诊断价值。方法:收集2012年3月—2016年8月本院消化内科住院患者,经CTE、内镜或病理证实为克罗恩病(CD)的患者31例,根据Harvey-Bradshaw指数(HBI)将患者分为活动期组(17例)和缓解期组(14例)。比较两组在CTE表现和C反应蛋白(CRP)指标间的差异。结果:活动期组较缓解期组有更大的小肠管壁厚度[(9.28±2.82)mm vs.(6.76±2.33)mm,P=0.012]、管壁强化程度[(111.47±9.8)HU vs.(91.50±9.55)HU,P=0.000]、CRP值[(48.56±31.03)mg/L vs.(20.24±19.29)mg/L,P=0.006]。活动期组梳征(14例)多于缓解期组(3例),差异具有统计学意义(P=0.001)。活动期组淋巴结增大、瘘管、蜂窝织炎及肠管狭窄的发生率(76.5%、23.5%、35.3%、100%)高于缓解期组(50.0%、7.1%、14.3%、85.7%),差异无统计学意义。结论:克罗恩病患者CTE上管壁的增厚程度、强化程度、强化方式、梳征对活动度的评估有重要指导作用。
[Abstract]:Objective: to evaluate the diagnostic value of CT enterography (CTE) in evaluating the activity of Crohn's disease. Methods: from March 2012 to August 2016, 31 inpatients with Crohn's disease confirmed by CTE, endoscopy or pathology were divided into active group (n = 17) and remission group (n = 14) according to Harvey-Bradshaw index. To compare the difference of CTE and C-reactive protein (CRP) between the two groups. Results: the thickness of small intestine wall in active group was greater than that in remission group [9.28 卤2.33 2.82)mm vs.(6.76 卤2.33 mm P0. 012], and the degree of wall enhancement [111.47 卤9.55 9.8)HU vs.(91.50 卤9.55 2.82)mm vs.(6.76 卤9.55] CRP value [48.56 卤19.29 mg / L P 0.006]. There were 14 cases of combing sign in active stage group than 3 cases in remission stage group, the difference was statistically significant. The incidences of lymph node enlargement, fistula, cellulitis and intestinal stenosis in active group were 76.53.35 and 100) higher than that in remission group 50.07.1and 85.75.70.There was no significant difference. Conclusion: the thickening degree, strengthening degree, strengthening mode and combing sign of upper wall of CTE in patients with Crohn's disease play an important role in the evaluation of mobility.
