乙肝肝纤维化与血管内皮功能相关性研究

【摘要】:目的:探讨慢性乙型肝炎(CHB)不同程度肝纤维化血管内皮功能差异。方法:收集2015年4月至2015年12月在新疆医科大学第一附属医院住院明确诊断CHB患者,根据肝组织学活检肝纤维化程度分为4组。使用Endo-PAT2000内皮功能检测仪测定外周动脉张力(PAT),并计算反应性充血指数(RHI)作为评价内皮功能指标;检测血生化指标,同时进行肝脏瞬时弹性成像测定肝脏硬度值(LSM),采用Pearson相关分析RHI与LSM的相关性。结果:收集到CHB患者61例,其中S0组8例,S1组22例,S2组20例,S3组11例。S0组、S1组、S2组、S3组患者的RHI、LSM、总胆固醇、低密度脂蛋白、丙氨酸氨基转移酶水平比较,差异有统计学意义(P0.05),S0组、S1组、S2组患者RHI为(1.93±0.30)、(1.78±0.38)和(1.63±0.16)高于S3组的(1.38±0.13)(P=0.00),S0组、S1组、S2组总胆固醇、低密度脂蛋白水平高于S3组差异有显著性(P0.05)。S3组丙氨酸氨基转移酶、LSM高于S0组、S1组、S2组差异有统计学意义(P0.01);患者RHI与LSM呈负相关(r=-0.323,P=0.011)。结论:在CHB患者随着肝组织肝纤维化程度增加RHI降低,肝脏瞬时弹性硬度测定LSM与RHI负相关,CHB患者随着肝纤维化程度增加低密度脂蛋白、总胆固醇降低。
[Abstract]:Objective: to investigate the difference of vascular endothelial function in chronic hepatitis B (CHB) with different degrees of hepatic fibrosis. Methods: CHB patients were collected from April 2015 to December 2015 in the first affiliated Hospital of Xinjiang Medical University. The patients were divided into 4 groups according to the degree of liver fibrosis biopsy. The peripheral arterial tension was measured by Endo-PAT2000 endothelial function tester, and the reactive hyperemia index (RHI) was calculated as an index to evaluate endothelial function. At the same time, the liver hardness was measured by transient elastography, and the correlation between RHI and LSM was analyzed by Pearson correlation. Results: 61 cases of CHB were collected, including 8 cases of S0 group, 22 cases of S 1 group, 20 cases of S 2 group, 20 cases of S 3 group, 11 cases of S0 group S 1 group and S 2 group S 3 patients. The levels of RHI LSM, total cholesterol, low density lipoprotein (LDL) and alanine aminotransferase (alt) were compared. There was significant difference in RHI between S0 group and S0 group (1.93 卤0.30, 1.78 卤0.38) and 1.63 卤0.16, respectively, which was higher than that in S3 group (1.38 卤0.13), and the total cholesterol in S _ 1 group was higher than that in S _ 1 group. The level of low density lipoprotein was significantly higher than that of S3 group. There was a significant difference in the level of LSM between S0 group and S0 group. There was a significant difference between LSM and S0 group, and there was a negative correlation between RHI and LSM. Conclusion: in patients with CHB, RHI decreased with the increase of liver fibrosis degree, and the total cholesterol decreased with the increase of LDL-C in patients with CHB and negative correlation between LSM and RHI.
