合用5-氨基水杨酸对炎症性肠病患者硫嘌呤类药物个体化应用的影响及其机制研究进展

【摘要】:目的:为临床合用5-氨基水杨酸(5-ASA)与硫嘌呤类药物治疗炎症性肠病(IBD)提供参考。方法:通过检索Pub Med、Web of Science、万方数据库、维普数据库和中国知网等相关专业数据库十多年来有关合用5-ASA对IBD患者应用硫嘌呤类药物的疗效和不良反应影响的研究文献,整理、归纳和综述二者合用对后者个体化应用的影响及其机制。结果与结论:5-ASA和硫嘌呤类药物是治疗IBD的常用药物,二者合用会升高后者活性代谢产物浓度,从而增强疗效,同时增大不良反应发生风险。其作用机制研究主要关注5-ASA对硫嘌呤类药物相关代谢酶和转运体活性的影响以及遗传因素与上述影响的相关性,但尚无统一结论。合用5-ASA对IBD患者硫嘌呤类药物个体化应用的影响及其机制仍有待于多中心、大样本的前瞻性研究加以证实。
[Abstract]:Objective: to provide a reference for the treatment of inflammatory bowel disease (IBD) with 5-aminosalicylic acid 5-ASA and thiopurine drugs. Methods: by searching for more than ten years related databases of Pub Med5-ASA, Wanfang database, Weipu database and China Zhiwang database, and other related databases, the therapeutic effect and adverse effects of 5-ASA on the efficacy and adverse reactions of thiopurine drugs in IBD patients were collected and sorted out. The effect and mechanism of the combination of the two on the individualized application of the latter are summarized and summarized. Results: the two drugs are commonly used in the treatment of IBD. The concentration of active metabolites of the latter can be increased by the combination of the two drugs, thus enhancing the efficacy and increasing the risk of adverse reactions. The effect of 5-ASA on thiopurine drug-related metabolic enzymes and transporter activities and the correlation between genetic factors and these effects were studied, but there was no uniform conclusion. The effect of 5-ASA on the individualized application of thiopurine drugs in IBD patients and its mechanism need to be confirmed by a large sample of prospective studies.
