2型糖尿病初诊患者肝脏脂肪含量的相关因素

【摘要】:目的探讨伴有非酒精性脂肪肝(NAFLD)的2型糖尿病患者肝脏脂肪含量与脂质代谢、胰岛素抵抗和胰岛β细胞功能的关系。方法采用3.0T质子磁共振波谱(1H-MRS)检测31例伴NAFLD的初诊2型糖尿病患者(观察组)肝脏脂肪含量,同时测量人体学参数和血清生化指标,口服75 g OGTT和胰岛素、C肽释放试验,用HOMA2评估胰岛素抵抗、β细胞功能,与健康体检证实血糖处于正常范围人员(对照组)进行比较,分析肝脏脂肪含量与脂质代谢、胰岛素抵抗和胰岛β细胞功能的关系。结果观察组BMI、WHR、ALT、TC、HDL-C、FPG、2h PG、空腹C肽、Hb A1C、HOMA2-IR、HOMA2-%B、HOMA2-%S及LFC与对照组比较显著升高(P0.05),TG、FINS两组间差异无统计学意义(P0.05)。多元逐步回归分析显示,BMI增加和HOMA2-IR升高是肝脏脂肪含量升高的独立危险因素。结论肝脏脂肪含量与肥胖、肝功能、胰岛素抵抗、胰岛β细胞功能相关,其中肥胖与胰岛素抵抗对肝脏脂肪含量影响最大。
[Abstract]:Objective to investigate the relationship between hepatic fat content, lipid metabolism, insulin resistance and islet 尾 cell function in type 2 diabetic patients with non alcoholic fatty liver disease (NAF LDD). Methods the liver fat levels in 31 newly diagnosed type 2 diabetic patients with NAFLD were measured by 3.0T proton magnetic resonance spectroscopy (1H-MRS). The anthropometric parameters and serum biochemical parameters were also measured. Oral administration of 75 g OGTT and insulin C peptide release test were performed. Insulin resistance, 尾 cell function were evaluated by HOMA2, and compared with those of normal blood glucose group (control group). The relationship between liver fat content and lipid metabolism, insulin resistance and islet 尾 cell function was analyzed. Results compared with the control group, there was no significant difference between the observation group and the control group in the two groups (P 0.05). The fasting C-peptide HbA1CU HOMA2-IRN HOMA2-S and LFC were significantly higher than those of the control group (P 0.05), and there was no significant difference between the two groups (P 0.05). Multiple stepwise regression analysis showed that the increase of HOMA2-IR and BMI were independent risk factors for the increase of liver fat content. Conclusion liver fat content is correlated with obesity, liver function, insulin resistance and islet 尾 -cell function, among which obesity and insulin resistance have the greatest influence on liver fat content.
