胰源性门脉高压症进展典型CT表现1例

【摘要】：正患者男,41岁,因"上腹胀痛1天"入院。肝功能、凝血、血常规、生化、大小便常规检查均未见异常。胃镜提示:胃底静脉重度曲张;食管静脉轻度曲张。腹部CTA提示:慢性胰腺炎并周围包裹性积液/假性囊肿形成;脾肿大。考虑患者存在门静脉高压症表现,拟行经颈静脉肝内门体分流术(TIPS)。追问病史,患者2年前有急性胰腺炎病史,曾有1次黑便,经保守治疗后好转。近两年来多次因腹痛住院治疗。回顾患者本院2014
[Abstract]:The patient, 41 years old, was admitted to hospital because of "upper abdominal distension pain for 1 day". Liver function, coagulation, blood routine, biochemistry, routine examination of urine and feces were not abnormal. Gastroscopy hint: gastric fundus vein severe varices; esophageal vein mild varices. Abdominal CTA suggests chronic pancreatitis with surrounding effusion / pseudocyst formation; splenomegaly. Considering the presence of portal hypertension, intrahepatic portosystemic shunt (tips) via jugular vein was proposed. The patient had a history of acute pancreatitis 2 years ago, had a black stool once, and improved after conservative treatment.
