布地奈德和美沙拉嗪对活动期炎症性肠病的疗效及作用机制研究

【摘要】：目的评价布地奈德和美沙拉嗪对活动期炎症性肠病的疗效及其对Th17细胞及白细胞介素-23(IL-23)的影响。方法 66例活动期炎症性肠病患者分为试验组和对照组各33例。对照组口服美沙拉嗪治疗,每次1g,每天4次。试验组在对照组的基础上吸入布地奈德200~1600μg·d-1联合治疗。治疗2个月后,对其临床疗效、Th17细胞水平及IL-23进行评价。结果试验组患者的治疗总有效率(93.93%)显著高于对照组(67.67%,P0.05)。试验组轻、中、重患者的Th17分别为(2.05±0.36),(2.59±1.09),(3.48±2.05),显著低于对照组的(2.33±0.51),(3.26±1.23),(4.67±2.27)(P0.05)。试验组轻、中、重患者的IL-23水平分别为(208.3±28.7),(619.4±34.5),(1276.8±59.7),显著低于对照组的(276.9±34.8),(698.3±47.8),(1356.3±71.6)(P0.05)。结论布地奈德和美沙拉嗪治疗活动期炎症性肠病效果良好,能有效降低Th17细胞和IL-23水平。
[Abstract]:Objective to evaluate the effects of budesonide and mesalazine on Th17 cells and interleukin-23 (IL-23) in patients with active inflammatory bowel disease. Methods 66 patients with active inflammatory bowel disease were divided into trial group (n = 33) and control group (n = 33). The control group was treated with mesalazine for 1 g each time, 4 times a day. The experimental group was treated with inhaled budesonide at 1600 渭 g d ~ (-1) on the basis of control group. After 2 months of treatment, the clinical efficacy of Th17 cells and IL-23 were evaluated. Results the total effective rate of treatment in the trial group (93.93%) was significantly higher than that in the control group (67.67%, P0.05). The Th17 of mild, moderate and severe patients in the trial group was (2.05 卤0.36), () 2.59 卤1.09), (3.48 卤2.05), significantly lower than that in the control group (2.33 卤0.51), (3.26 卤1.23), (4.67 卤2.27) (P0.05). The IL-23 levels of mild, moderate and severe patients in the trial group were (208.3 卤28.7), (卤34.5), (1276.8 卤59.7), significantly lower than those in the control group (276.9 卤34.8), (698.3 卤47.8), (1356.3 卤71.6) (P0.05). Conclusion budesonide and mesalazine are effective in the treatment of active inflammatory bowel disease and can effectively reduce the level of Th17 cells and IL-23.
