失代偿期原发性肝硬化患者合并血流感染的临床研究

【摘要】：目的探究失代偿期原发性肝硬化患者合并血流感染的临床特征、特点。方法选取2013年5月-2016年5月于医院接受治疗的1043例失代偿期原发性肝硬化患者,回顾性调查患者的一般资料发病率及感染患者体征、病发相关因素,同时对患者感染血样进行培养,诊断其病原菌,调查患者中性粒细胞以及白细胞数目及预后情况。结果失代偿期肝硬化合并血流感染患者112例,感染率为10.73%,有发热36~38℃29例占27.35%,发热38℃43例占40.57%,畏寒56例占52.83%。住院次数和曾接受侵袭性操作与血流感染有相关性(P0.05);培养病原菌共110株,其中革兰阴性菌69株占62.73%,以大肠埃希菌为主;革兰阳性菌41株占37.27%,金黄色葡萄球菌为主,两种及两种以上病原菌感染14例占13.21%;失代偿期肝硬化合并血流感染患者中性粒细胞数目比例0.75有87例占82.08%,白细胞数目(10.96±2.12)×109/L,普通肝硬化患者中性粒细胞数目比例0.75有123例占13.21%,白细胞数目(7.89±2.38)×109/L,差异具有统计学意义(P0.05);预后疗效中,失代偿期肝硬化合并血流感染患者有效59.43%,无效40.56%,死亡率11.32%。结论失代偿期肝硬化患者合并血流感染发病率高,预后效果差,且发病特征不明显;减少住院次数和侵袭性操作,保证手术无菌环境有利于预防血流感染;以中性粒、白细胞进行初期检验比血样培养结果耗时短,能尽早进行初期预后,配合血样培养的准确结果,有助于提早诊断时间。
[Abstract]:Objective to investigate the clinical features and characteristics of blood flow infection in patients with decompensated primary liver cirrhosis. Methods 1043 patients with decompensated primary liver cirrhosis who were treated in hospital from May 2013 to May 2016 were investigated retrospectively. At the same time, the infected blood samples were cultured, the pathogenic bacteria were diagnosed, and the neutrophil and leukocyte count and prognosis were investigated.
