狼疮相关性胰腺炎的临床研究进展

【摘要】：背景:狼疮相关性胰腺炎在临床较为少见,但其病情复杂、预后欠佳,在临床诊疗过程中容易发生漏诊和误诊,目前,关于此病的认识大多来源于病案报道及小样本回顾研究,对于该病的发病机理、临床表现及治疗方法也缺乏共识,该病对于全身器官及系统的损害使其成为多学科治疗的难题。目的:通过综述目前对狼疮性胰腺炎的相关临床研究进展,从而为临床医生更好的认识及治疗此疾病提供参考及帮助。方法:全面检索发表在中国期刊全文数据库、中国优秀硕士、博士学位论文数据、中国重要会议论文全文数据库、中国生物医学文献数据库、中国知网数据库、中国万方数据库、中国维普数据库、PubMed、Medline、Ovid、Embase、Cochrane Library数据库有关系统性红斑狼疮与胰腺炎临床研究进展的所有中英文文献,并总结该疾病的相关临床研究进展。结果与结论:狼疮相关性胰腺炎是一种与狼疮活动密切相关的胰腺炎症性病变,虽然该病的年发病率仅为0.4—1.1‰,但其死亡率却高达37%,其中重症患者高达75%。狼疮性胰腺炎的病情复杂,无明显特异性临床表现,故一经诊断需早期干预,如出现胰腺出血坏死、多器官受累、严重感染等情况时,往往提示预后不良,更需严密监测,积极治疗,早期给予中至大量的激素及免疫抑制剂对于狼疮性胰腺炎是安全有效的,其并非是狼疮性胰腺炎的主要病因,亦不会加重病情,反而有利于病情缓解,改善预后。
[Abstract]:Background: lupus associated pancreatitis is rare in clinical practice, but its condition is complex and its prognosis is poor. It is easy to miss diagnosis and misdiagnosis in the course of clinical diagnosis and treatment. At present, most of the knowledge about this disease comes from medical record report and small sample retrospective study. There is a lack of consensus on the pathogenesis, clinical manifestations and treatment of the disease, and the damage to the whole body organs and systems makes it a difficult problem for multidisciplinary treatment. Objective: to review the clinical research progress of lupus pancreatitis so as to provide reference and help for clinicians to better understand and treat this disease. Methods: a comprehensive search was made for the data published in the full text database of Chinese journals, the excellent master's degree, the doctoral degree thesis, the full text database of important conference papers in China, the Chinese biomedical literature database and the China knowledge network database. All the Chinese and English literatures on the progress of clinical research on systemic lupus erythematosus and pancreatitis were reviewed in China Wanfang Database, China Wiper Database and PubMed,Medline,Ovid,Embase,Cochrane Library Database, and the related clinical research progress of this disease was summarized. Results & conclusion: lupus associated pancreatitis is a kind of pancreatic inflammatory disease closely related to lupus activity. Although the annual incidence of lupus is only 0.4-1.1 鈥，
