非酒精性脂肪肝对新发糖尿病的影响

【摘要】:目的探讨非酒精性脂肪肝(NAFLD)对新发糖尿病的影响。方法采用前瞻性队列研究,以参加2006年7月-2007年10月开滦集团公司健康体检的离退休职工为研究对象,排除既往有糖尿病病史、饮酒史以及资料不全者,最终44 019例纳入队列。对所有参与者进行问卷调查、体格检查和实验室检查,每两年随访一次。将研究对象按基线有无NAFLD分为NAFLD组和非NAFLD组,比较两组新发糖尿病检出率和累积发病率,采用多因素Cox回归模型分析NAFLD对新发糖尿病的影响。结果在全人群中,NAFLD组新发糖尿病检出率和累积发病率(分别为12.3%、12.5%)均高于非NAFLD组(分别为4.5%、4.7%),差异有统计学意义(P0.01)。多因素Cox回归模型结果显示,在校正年龄、性别、腰围等因素后,NAFLD组发生糖尿病的风险是非NAFLD组的1.61倍(95%CI 1.47~1.75)。结论基线NAFLD是新发糖尿病的独立危险因素。
[Abstract]:Objective to investigate the effect of non-alcoholic fatty liver (NAFLD) on new diabetes mellitus. Methods A prospective cohort study was conducted among retired workers who participated in the physical examination of Kailuan Group Company from July 2006 to October 2007. The previous history of diabetes mellitus, drinking history and incomplete data were excluded. In the end, 44 019 cases were added to the queue. All participants were followed up every two years by questionnaire, physical examination and laboratory examination. The subjects were divided into NAFLD group and non-NAFLD group according to the presence or absence of NAFLD at baseline. the detection rate and cumulative incidence of new diabetes mellitus were compared between the two groups. The effect of NAFLD on new diabetes mellitus was analyzed by multivariate Cox regression model. Results the detection rate and cumulative incidence of new diabetes mellitus in NAFLD group (12.3%, 12.5%) were significantly higher than those in non-NAFLD group (4.5%, 4.7%) (P 0.01). The results of multivariate Cox regression model showed that after adjusting for age, sex, waist circumference and other factors, the risk of diabetes in NAFLD group was 1.61 times higher than that in non-NAFLD group (95%CI 1.47 鈮,
