恩替卡韦治疗乙肝病毒相关慢加急性肝功能衰竭的临床研究

　　[背景]乙型病毒性肝炎相关慢加急性肝功能衰竭患者预后差病死率高。大多数预测肝衰竭预后的模型计算复杂且敏感性低。恩替卡韦治疗乙型病毒性肝炎相关慢加急性肝功能衰竭报道较少。[目的]探讨恩替卡韦治疗乙型病毒性肝炎相关慢加急性肝功能衰竭的安全性和有效性。建立预测乙型病毒性肝炎相关性慢加急性肝功能衰竭患者预后的预测模型（Tongji prognostic predictor model, TPPM）。[方法]共有248例乙型病毒性肝炎相关慢加急性肝功能衰竭患者纳入本回顾性研究。其中124例在内科标准治疗的基础上加用恩替卡韦,另外124例患者只进行内科标准治疗但不使用核苷类似物抑制病毒。抗病毒治疗组和对照组在基线病毒学、临床生化等均无显著差异。收集患者生存期、生化指标、凝血功能、血液常规检查、血清学指标、病毒学指标和并发症等临床数据,通过多因素回归分析建立预测预后的回归模型—同济预后预测模型（Tongji prognostic predictory model, TPPM）。[结果]恩替卡韦治疗组患者1月和3月生存率分别为72.58%（90/124）和61.29%（76/124）,显著高于对照组的...
