36例腐蚀性上消化道狭窄病例的回顾性研究

背景随着酸性、碱性化合物以日常生活用品的成分走近人们的身边,人们接触腐蚀性物质的几率越来越大。虽然一再强调这些生活用品的安全性及注意事项,无意或故意摄入腐蚀性物质的事件却一直在发生。腐蚀性物质摄入的发病率因国家和文化背景而异,目前无法从现有的文献及病例报道中得知其真实发病率,尤其在发展中国家。碱性和酸性腐蚀剂是最常见的摄入化学物质,可以通过凝固或液化反应产生组织破坏并引起消化道损伤。腐蚀性损伤的临床表现多种多样,从没有粘膜损伤到严重损伤。急性期可发生穿孔和坏死,远期的并发症包括消化道的狭窄和恶性肿瘤的发生。与儿童一般是误服腐蚀性物质不同,成年人摄入主要是有自杀倾向或精神疾病所致。上消化道狭窄是腐蚀性物质摄入后最常见的远期并发症,其中食管狭窄占大多数,狭窄的严重程度和范围取决于所摄入腐蚀性物质的物理形式、种类、数量、浓度、粘膜接触时间和摄入时脏器状态。腐蚀性狭窄的治疗旨在改善症状/体征、避免消化道梗阻并发症并防止狭窄复发。狭窄扩张术是目前应用最广泛的治疗方法,包括球囊扩张术和探条扩张术,既可在内镜下进行,也可在X线透视下或两者联合下进行。与其他良性消化道狭窄相比,腐蚀性狭窄常为多发的、长...
