乙肝相关性肝病肝脏内干细胞样细胞的特性和定向分化

前言目前全球有3.5亿乙型肝炎病毒(hepatitis B virus,HBV)慢性携带者,每年约1百万死于HBV感染所致的肝衰竭、肝硬化、原发性肝癌。中国属HBV感染高流行国家,约有1.2亿慢性HBV携带者和3000万慢性乙型肝炎患者,这些慢性乙型肝炎患者很容易发展成肝硬化、肝癌。因此,慢性乙型肝炎是严重威胁人类健康的疾病之一,但是HBV的精确致病机制尚未完全阐明。尽管外科技术和各种辅助治疗手段的完善,但是肝脏的功能衰竭和肝癌手术切除后的复发和转移仍然是乙肝相关性肝病的主要死亡原因。因此,乙型肝炎病毒慢性感染导致的肝脏纤维化、肝硬化甚至肝细胞肝癌仍然是基础和临床研究的热点。尽管在肝脏病毒感染后所致的肝脏纤维化在一定的条件下可以实现逆转,但是目前的研究结果仅仅局限于动物模型中,尤其是在晚期肝硬化形成后,肝脏的病理改变已无法逆转。众所周知,人体多种组织器官中存在干细胞,这些细胞在器官损伤时是组织再生和修复。研究认为,组织器官中的干细胞与肿瘤细胞有着密切的关系且具有某些共同特征。最近的研究表明,组织器官中的成体干细胞可能是组织细胞发生恶性转化的靶细胞。在发生组织损伤的器官中,慢性感染和组织...
