SD大鼠血清心肌损伤标志物的死后变化及法医学应用

【摘要】:目的:通过对不同死因(溺死、以心肌损伤为主)大鼠死后不同时间心肌损伤标志物含量变化进行研究,分析不同死因大鼠死亡后心肌损伤标志物的变化规律,完善心肌损伤标志物在死后检测的法医学意义。方法:选取SD大鼠75只,体重为275g±25克,随机分为3组(n=25):胸部打击组采用改进BIM-II型生物撞击机闭合式打击大鼠心前区,打击力度为120Kpa;溺死组;空白对照组脱颈处死。各组大鼠各分为五个亚组(n=5),分别于死亡后6h、12h、24h、48h、72h取右心血液并制备成血清,对心肌酶谱五项及肌钙蛋白进行检测,同时提取心肌组织进行HE染色;计量资料以均数±标准差表示,采用单因素方差分析,对比三组之间差别,p0.05时,其差异具有统计学意义;对每组死亡后经历时间与心肌损伤标志物进行线性回归,R2≥0.8认为心肌损伤标记物与时间之间具有线性关系。结果:(1)采用BIM-II型生物撞击机闭合式打击大鼠心前区建立SD大鼠心脏挫伤模型成功。(2)除48小时的CK,6小时的LDH,72小时的GPT外,对照组和溺死组心肌损伤标志物之间的差异无统计学意义(P0.05);CK-MB三组两两之间无统计学意义(P0.05),c Tn T、CK、GOT、LDH几项指标在打击组与空白组,打击组与溺死组之间大部分时间点上,其差异无统计学意义(P0.05);GPT上,打击组和空白组,打击组和溺死组大部分时间点上差异具有统计学意义(P0.05)。对同一死因不同时间点的组内对比发现,三组大鼠死后6h和其他时间点大部分差异具有统计学意义(P0.05),其他时间点上大部分心肌损伤标志物之间的差异无统计学意义(P0.05)。(3)空白对照组大鼠各项心肌损伤标志物中,c Tn T,LDH的含量与时间呈线性关系(R20.8);胸部打击组中,c Tn T,CK-MB的含量与时间呈线性关系(R20.8);溺死组中,LDH的含量与时间呈线性关系(R20.8);其余指标在各组中,含量和时间均不呈线性关系。在呈线性关系的心肌损伤标志物中,空白对照组c Tn T在含量是随着死亡时间的增加而降低的(呈负相关),而在胸部打击组中c Tn T的含量却随着死亡时间的增加而增加(正相关)。结论:(1)采用BIM-II型生物撞击机闭合式打击大鼠心前区建立SD大鼠心脏挫伤模型是成功的,可用于心肌挫伤的系列研究。(2)文中涉及的三种死因死亡的尸体,在20℃、湿度78%条件下保存,3天内细胞结构仍可辨认,可用于形态学诊断。(3)心肌损伤标志物在死亡后12-72小时内无统计学差异,故心肌损伤标志物在死亡后3天内可以作为心肌损伤的辅助诊断指标之一。(4)不同死因死亡的SD大鼠血清心肌损伤标志物随死后经历时间变化呈现不同趋势,通过血清心肌损伤标志物推断死亡时间需考虑死因的影响。
[Abstract]:Objective: to study the changes of myocardial injury markers at different time after death in rats with different causes of death (drowning, mainly myocardial injury), and to analyze the changes of myocardial injury markers in rats with different causes of death. Methods: 75 SD rats were selected and weighed 275 g 卤25 g.
