胰腺外伤和相关并发症及其在随访复查中的MSCT表现

【摘要】：目的总结胰腺外伤和相关并发症及其在随访复查中的多层螺旋CT(MSCT)影像表现。方法回顾性分析28例经临床确诊的胰腺外伤患者MSCT检查资料,参考美国创伤外科学分会(AAST)提出的分级标准,对胰腺损伤进行分级诊断,观察胰腺不同损伤等级和相关并发症及其在随访复查中的MSCT影像表现。结果首诊时漏诊3例,25例首诊时可见胰腺损伤的直接征象而明确诊断,28例可见胰腺损伤间接征象。Ⅰ级损伤8例,Ⅱ级损伤6例,Ⅲ级损伤7例,Ⅳ级损伤5例,Ⅴ级损伤2例。增强扫描能清晰显示胰腺损伤范围及断裂带,断裂带超过50%者术中可见主胰管断裂伤。胰腺损伤相关并发症包括假性囊肿9例,胰周血管损伤6例,腹腔或胰周感染2例,胰腺萎缩3例。结论 MSCT能及时发现胰腺损伤,增强扫描可明显提高胰损伤诊断的阳性率,更准确地评估胰腺损伤程度及分级。MSCT随访可观察胰腺恢复情况,有无假性囊肿形成及囊肿的大小变化、腹腔或胰周感染、胰腺萎缩等并发症,为临床医生选择合理的治疗方案提供依据。
[Abstract]:Objective to summarize the pancreatic trauma and related complications and the MSCT of multislice spiral CTT in the follow-up and reexamination of pancreatic trauma. Methods the MSCT findings of 28 patients with pancreatic trauma were retrospectively analyzed. According to the classification standard proposed by AAST, the classification diagnosis of pancreatic injury was carried out. To observe the different grades of pancreatic injury and related complications and their MSCT imaging findings in follow-up and reexamination.
