40ml造影剂在256层螺旋CT头颈部动脉造影中的应用

【摘要】:目的: 头颈部CTA已经成为评估颈部及颅内血管病变的重要检查方法之一。然而头颈部CTA检查过程中,50ml造影剂用量常会出现静脉周围伪影,本研究旨在探讨256层螺旋CT头颈部造影检查时,不同剂量造影剂的成像效果。 材料与方法: 66例患者随机分为两组,每组患者33人。所有患者行头颈部CTA检查。A组患者(男21例,女12例;平均年龄56.87±11.53岁)常规注射50ml造影剂,B组患者(男19例,女14例;平均年龄56.94±11.51岁)注射40ml造影剂,,两组患者注射造影剂的速度均为5ml/s,注射造影剂后均追加生理盐水40ml。客观评价通过测量动脉的CT值来评估,测量位置动脉选取10处,包括颈内动脉7处、椎基底动脉3处,静脉选取4处。主观评价由两名放射科医师独立完成,通过评估轴位图像及最大密度投影图像,采用4分法评估。统计学处理采用t检验和Mann Whitney U检验,P<0.05有统计学意义。 结果: 图像质量的客观评价中,两组患者头颈部动脉强化CT值无统计学差异,颈动脉的平均CT值无统计学差异(p=0.87),椎基底动脉的平均CT值无统计学差异(p=0.72)。B组患者静脉强化CT值较A组显著降低(110±19.61HUvs.188.27±57.4HU;p0.001)。图像质量的主观评价中,B组患者锁骨下静脉周围的伪影较A组降低(p0.001),动脉图像的评价无显著性差异。 结论: 256层螺旋CT头颈部造影中,与50ml造影剂用量比较,40ml造影剂用量的动脉图像质量无统计学差异,但显著降低了锁骨下静脉周围伪影。
[Abstract]:Objective:. Head and neck CTA has become one of the most important methods to evaluate the cervical and intracranial vascular lesions. However, in the CTA examination of the head and neck, the venous artifacts often appear in the dosage of 50ml contrast medium. The purpose of this study was to investigate the imaging effects of different doses of contrast media in 256-slice spiral CT cephalographic examination. Materials and methods:. 66 patients were randomly divided into two groups, 33 patients in each group. All patients received CTA examination of head and neck. Group A (21 males, 12 females; mean age 56.87 卤11.53 years) was given 50ml contrast agent routine injection (19 males, 14 females). The average age was 56.94 卤11.51 years old) 40 ml contrast agent was injected. The speed of injection of contrast agent was 5 ml / s in both groups, and 40 ml saline was added to the contrast medium. The objective evaluation was evaluated by measuring the CT value of the artery, and 10 arteries were measured. There were 7 internal carotid arteries, 3 vertebrobasilar arteries and 4 veins. The subjective evaluation was performed by two radiologists independently. Statistical analysis was performed with t test and Mann Whitney U test (P < 0. 05). Results:. In the objective evaluation of image quality, there was no significant difference in CT value of head and neck artery enhancement between the two groups. There was no significant difference in the average CT value of carotid artery and vertebrobasilar artery. The enhanced CT value of vein in group B was significantly lower than that in group A (110 卤19.61 HUvs.188.27 卤57.4 HUP 0.001). The subjective evaluation of image quality in group B was around the subclavian vein. The artifact in group A was lower than that in group A (P 0.001), and there was no significant difference in the evaluation of arterial images. Conclusion:. There was no significant difference in arterial image quality between 256-slice spiral CT and 50ml contrast medium dosage, but the artefact around the subclavian vein was significantly decreased.
