320层CT前瞻性门控窄窗和靶向扫描方式在低心率冠状动脉成像中的辐射剂量、图像质量对照研究

【摘要】:目的:比较前瞻性心电门控窄窗扫描与靶向扫描方式在320层冠状动脉成像中的辐射剂量、图像质量。方法:对临床拟诊冠心病、心率65次/min的200例患者依次分别采用前瞻性门控窄窗扫描和靶向扫描方式冠状动脉成像扫描方案,分为P组(Prospect-ive CTA前瞻组,70%~80%的窄曝光窗)和T组(Target CTA靶向组,固定期相75%曝光窗),评价两组的辐射剂量、图像质量及诊断结果。结果:P组和T组各有1500(15×10 0)个冠状动脉节段,P组和T组冠状动脉节段管径太小(1.5m m)难以评估分别占3.53%(53/150 0)、3.8%(57/1500),可评估节段占96.47%(1447/1500)、96.2%(1443/1500),差异无统计学意义(P0.05);在可评估冠状动脉节段中,P组的1分、2分、3分及4分图像分别占98.82%(1426/1443)、0.9%(13/1443)、0.27%(4/1443)及0,T组的为95.02%(1376/1447)、1.93%(28/1447)、2.41%(35/1447)及0.55%(8/1447)。P组和T组的图像评分差异有统计学意义(P0.05)。P组和T组平均辐射剂量为(2.36±0.21)m Sv、(2.31±0.2)m Sv,P组和T组的平均辐射剂量差异无统计学意义(P0.05)。结论:320层CT在低心率冠状动脉成像中前瞻性门控窄窗扫描的图像质量及扫描成功率优于靶向扫描。前瞻性门控窄曝光窗扫描可替代靶向扫描。
[Abstract]:Aim: to compare the radiation dose and image quality of prospective ECG gated narrow window scan and target scan in 320 slice coronary artery imaging.Methods: 200 patients with clinically suspected coronary heart disease (CHD) and heart rate (HR) 65 / min were examined with prospective gated narrow window scan and targeted coronary angiography respectively.Group P was divided into Prospect-ive CTA prospective group (70% narrow exposure window) and T group (target CTA target group, fixed phase 75% exposure window).
