锥形束CT对牙槽嵴裂植骨术后的评价

【摘要】:目的:应用锥形束CT(cone beam computed tomography CBCT)对牙槽嵴裂植骨(ABG)患者术后患者不同时间段骨密度(BMD)及骨高度观测,了解骨愈合稳定时期及植骨区唇腭侧骨量变化,为进行后序治疗时机提供参考。 方法:选取20名单侧完全性牙槽突裂患者,常规行牙槽嵴裂髂骨移植术,术后3个月、6个月复查,ProMax3D检查获得原始数据以DICOM格式存放,图像输入Simplant11.04软件重建。测量术后3个月、6及健侧骨密度值,结果用Hounsfieldunits (HU)表示;根据改良Bergand分类法对术后3个月、6个月植骨区唇、腭侧骨高度分别进行分类,并与全景片分类对比。 结果:术后3个月平均骨密度(BMD)(385.086±73.572HU)与术后6个月(356.380±78.297HU),无明显差异(P0.05);均小于健侧(475.475±65.826HU)(P0.05)。术后3个月唇、腭侧骨高度分类与同月全景片分类比较有差异(P0.05);术后6个月唇、腭侧骨高度分类与同月全景片分类比较有差异(P0.05);术后3个月唇、腭侧及全景片骨高度分类分别与术后6个月唇、腭侧及全景片骨高度分类比较有差异(P0.05) 结论:本研究中牙槽突裂植骨术后3个月与6个月骨密度均值无明显差异;唇侧与腭侧分类不同,且术后6个月骨高度分类较术后3个月低。说明骨吸收持续存在,术后3个月骨密度稳定,提示临床医生可在术后3个月进行后序治疗;骨移植区不同部位骨量不同,唇侧或者腭侧存在全景片不易发现的骨凹陷, CBCT可客观评价植骨区骨质量,对手术评价及指导后序治疗有临床应用价值。
[Abstract]:Objective: to observe the bone mineral density (BMD) and bone height in patients with alveolar cleft bone graft (ABG) at different time after operation by using conical bundle CT(cone beam computed tomography CBCT), and to understand the changes of bone mass in the labiopalatine side during the stable period of bone healing and bone grafting. To provide a reference for the timing of post-sequence therapy. Methods: 20 patients with complete alveolar cleft were selected and underwent alveolar crest iliac bone transplantation. After 3 months and 6 months, the original data were stored in DICOM format and the images were reconstructed by Simplant11.04 software. Bone mineral density (BMD) was measured 3 months after operation, and the results were expressed by Hounsfieldunits. According to the modified Bergand classification, the lip and palatal bone height were classified in 3 months and 6 months after operation, and compared with panoramic radiographs. Results: there was no significant difference in BMD between 3 months after operation (385.086 卤73.572HUU) and 6 months after operation (356.380 卤78.297HUU). There were significant differences in the classification of lip and palatine bone height between the same month and the same month after operation (P0.05), the classification of palatine lateral bone height in 6 months after operation was different from that in the same month (P0.05), and the third month after operation was compared with that of panoramic film (P0.05). The classification of bone height of palatal side and panoramic film was different from that of lip, palatine side and panoramic bone height of 6 months after operation (P0.05). Conclusion: in this study, there was no significant difference in BMD between 3 months and 6 months after alveolar fissure grafting, and the classification of lip and palate was different, and the classification of bone height at 6 months after operation was lower than that at 3 months after operation. This indicates that bone resorption persists and bone mineral density is stable at 3 months after operation, suggesting that clinicians can carry out posterior treatment at 3 months after operation, and bone mass in different parts of bone graft area is different. CBCT can objectively evaluate the quality of bone in the bone graft area, and it has clinical application value in the evaluation of surgery and the guidance of post-sequence therapy.
