复杂型先天性心脏病的双源CT检查与评价

【摘要】:目的以手术或CAG结果为标准,比较DSCT心血管成像及TTE对复杂先天性心脏病的诊断及临床应用价值。资料及方法对我院2009年9月至2012年9月所收治的33例(其中男性23例,女性10例)CCHD的病例资料进行回顾性分析,所有病例均行DSCT及TTE检查,,其中18例经手术证实,2例经CAG证实。结果33例患者经DSCT共检出150处畸形,TTE共检出126处。20例患者经手术或CAG证实畸形共93处,DSCT及TTE分别检出89、74处,诊断准确率分别为95.7%、79.6%,两者差异有统计学意义(χ~2=10.3,p<0.05)。其中心内畸形共27处, DSCT及TTE分别检出26、27处,诊断准确率分别为96.3%、100%,两者差异无统计学意义(χ~2=0,p>0.05);心脏与大血管连接部分畸形共11处,DSCT及TTE分别检出11、10处,诊断准确率分别为100%、90.9%,两者差异无统计学意义(χ~2=0,p>0.05);心外畸形共55处,DSCT及TTE分别检出52、37处,诊断准确率分别为94.5%、67.3%,两者差异有统计学意义(χ~2=11.5,p<0.05)。若将DSCT与TTE联合应用,则诊断准确率可达97.8%。结论DSCT在诊断复杂先心病心外畸形方面较TTE更具优势,可作为其补充检查手段。术前将两者联合应用,可进一步提高复杂先天性心脏病的诊断准确率。
[Abstract]:Objective to compare the diagnostic and clinical value of DSCT and TTE in complex congenital heart disease with surgical or CAG results. Materials and methods the data of 33 cases (23 males and 10 females) admitted to our hospital from September 2009 to September 2012 were retrospectively analyzed. All cases were examined by DSCT and TTE, of which 18 cases were confirmed by CAG. Results one hundred and fifty deformities were detected by DSCT in 33 patients. Twenty patients were proved by operation or CAG to have 93 deformities. The diagnostic accuracy was 95.7% and 79.6%, respectively. The difference between them was statistically significant (蠂 ~ (2 +) 10.3 p < 0.05 ~ (5) P), and the diagnostic accuracy was 95.7% and 79.6% (蠂 ~ (2) 210.3 p < 0.05) respectively, and the accuracy of diagnosis was 99.7% and 79.6% respectively (蠂 ~ 2 / 10 ~ (-3) P < 0.05). There were 27 deformities in the center, 2627 in DSCT and 27 in TTE. The diagnostic accuracy was 96.3 and 100, respectively. There was no significant difference between the two groups (蠂 ~ 2 / 0 / 0, P > 0.05), and there were 11 cases of cardiac and large vascular junction malformations, 11 cases were detected by DSCT and TTE, respectively. The accuracy of diagnosis was 90.9 and there was no significant difference between the two groups (蠂 ~ 2, P > 0.05), and 55 cases of extracardiac malformation were detected by DSCT and TTE, respectively, and the diagnostic accuracy was 94.5% and 67.3%, respectively (蠂 ~ 2 11.5p < 0.05). If DSCT and TTE are combined, the diagnostic accuracy can reach 97.8%. Conclusion DSCT is superior to TTE in the diagnosis of complex congenital heart disease and can be used as a supplementary examination method. Preoperative combined application can further improve the diagnostic accuracy of complex congenital heart disease.
