乳腺X线与自动乳腺容积超声检查对病变检出效能的比较

【摘要】:目的:探讨乳腺X线(mammography)与自动乳腺容积超声(automated breast ultrasound system,ABUS)检查对病变的检出效能。方法:收集2016年7月至2016年9月142例天津医科大学肿瘤医院经术后病理证实的乳腺良恶性病变患者的临床资料,共149个病变纳入研究,比较术前患者乳腺X线和ABUS检查对病变的检出率。结果:乳腺X线和ABUS检查对病变总检出率分别为87.2%(130/149)和98.0%(146/149),两者之间差异具有统计学意义(P0.05)。乳腺X线和ABUS检查对乳腺癌的检出率分别为91.1%(92/101)和97.0%(98/101),两者之间差异无统计学意义(P0.05);乳腺X线和ABUS检查对良性病变的检出率分别为79.2%(38/48)和100.0%(48/48),两者之间差异具有统计学意义(P0.05)。ABUS在致密型乳腺中的病变总检出率及乳腺癌、良性病变检出率分别为97.7%(126/129)及96.5%(82/85)、100.0%(44/44),乳腺X线检查分别为86.0%(111/129)及90.6%(77/85)、77.3%(34/44),差异均具有统计学意义(P0.05)。乳腺X线检查漏诊的为乳腺致密组织和腺体深面的病变,ABUS检查漏诊的多为钙化型病变。结论:ABUS检查对病变总检出效能及致密型乳腺中的病变检出效能均高于乳腺X线检查,漏诊的病变以钙化为主要影像学表现,乳腺X线检查对钙化型病变的检出具有独特优势。乳腺X线与ABUS检查在乳腺癌筛查中具有一定的互补作用。
[Abstract]:Objective: to investigate the effectiveness of mammography and automated breast ultrasound system ABUSS in detecting lesions. Methods: from July 2016 to September 2016, 142 patients with benign and malignant breast lesions confirmed by pathology were collected from Cancer Hospital of Tianjin Medical University, and 149 lesions were included in the study. To compare the detection rate of breast X-ray and ABUS before operation. Results: the total detection rate of mammography and ABUS were 87.2 / 149 and 98.0 / 146 / 149 respectively. The difference between them was statistically significant (P 0.05). The detection rates of mammography and ABUS for breast cancer were 91.2 / 101) and 97.0 / 98 / 101, respectively. The difference was statistically significant (P0.05). Breast X-ray examination missed the lesions of dense breast tissue and deep gland. ABUS was mostly calcified lesions. Conclusion the detection efficiency of the 7% ABUS examination is higher than that of the breast X-ray examination. Calcification is the main imaging manifestation in the missed lesions, and mammography has a unique advantage in the detection of calcified lesions. Mammography and ABUS are complementary to each other in breast cancer screening.
