血管内皮细胞损伤与放射性肺损伤的相关实验研究

【摘要】:目的:探讨大鼠肺组织受照射后,血管内皮细胞损伤导致放射性肺损伤发生、发展的可能致病机理。 方法:本研究选用60Coγ射线治疗机单次照射大鼠右肺,照射剂量分别为OGy、7.OGy、14.4Gy,建立大鼠放射性肺损伤模型,选择照射后1天(以下以d表示)、7d、30d、90d和180d为观察时间点,取对照组及各照射组大鼠的肺组织进行相关检测,包括:①大鼠肺放射性损伤的病理学观察,包括肉眼观察、H-E染色及masson染色,光学显微镜观察;②免疫组织化学方法检测肺组织CD34、CD105、VEGF和FⅧ:RAg的表达水平。 结果:(1)肉眼观察:对照组大鼠肺组织淡红色,表面光滑,弹性好,7.0Gy组未见明显的出血、水肿及纤维化改变;14.4Gy组早期可见片状出血、水肿和肿胀,照射后90d,肺组织暗红色,肺组织表面有白色斑点或斑片状,照射后180d,大部分肺组织表面呈灰白色,肺体积缩小,肺表面凹凸不平,弹性明显降低、质地较硬,部分肺叶塌陷。(2)光镜下病理组织学所见:对照组肺组织结构正常;7.0Gy组照射后早期(1d、7d),肺泡腔及间隔可略见轻微渗出、充血和水肿;14.4Gy组肺泡腔及间隔可见渗出、充血、水肿和炎性细胞浸润,局部可见红细胞渗出;照射后30d,7.0Gy组肺泡腔及间隔出现轻微水肿和炎性细胞浸润;14.4Gy组见肺泡间隔增宽、水肿、炎性细胞浸润和出血,以及肺泡间隔及部分肺泡腔内纤维素样的渗出物,并且可见胶原形成;照射后90d,14.4Gy组肺泡间隔进一步增宽,局部仍见出血,部分肺泡腔塌陷、不张、肺大泡形成并且出现胶原形成;照射后180d,14.4Gy组出现明显的胶原形成和纤维化改变。(3)免疫组织化学检测:①CD34,与正常对照组比较,14.4Gy组除1d外,其余各观察时点均有显著差异(P0.05),而7.0Gy组各时间点均无显著性差异(P0.05);②CD105,与正常对照组比较,7.0Gy组各时点均无显著差异(P0.05);14.4Gy组除1d,90d外,其余各观察时点均有显著差异(P0.05);③VEGF,与正常对照组比较,7.0Gy组无显著差异(P0.05);14.4Gy组,除90d、180d有显著差异(P0.05)外,其余时点均无显著差异(P0.05);④FVⅢ:RAg,与正常对照组比较,14.4Gy组从1d开始,各时点均有显著差异(P0.05),而7.0Gy组各时间点均无显著性差异(P0.05)。 结论:肺组织放射损伤后,微循环中成熟内皮细胞数量的进行性减少,是新生毛细血管生成障碍,纤维性修复慢性迁延的根本原因。
