婴幼儿正常内耳结构的HRCT测量

【摘要】:目的人工耳蜗植入术(CI)前需评价内耳情况,婴幼儿正常内耳结构的HRCT测量为评价内耳发育提供影像依据。资料与方法前瞻性选取正常颞骨HRCT图像129例(258耳),按年龄分为A组92例(184耳,1~12个月),B组21例(42耳,13~24个月),C组16例(32耳,25~36个月)。测量耳蜗顶中周高度、耳蜗底周长径、耳蜗底周内径、耳蜗神经管(CNC)宽径、耳蜗导水管(CA)长径、CA外口宽径、外半规管(LSCC)骨岛直径,统计LSCC骨岛中心透亮影的阳性率。结果不同年龄、性别和侧别间的耳蜗底周长径、管内径、顶中周高度、CNC宽径及LSCC骨岛宽径相比,差异无统计学意义(P0.05);不同性别和侧别间的CA长径及外口宽径相比,差异无统计学意义(P0.05),但随年龄增长而增大。不同性别和侧别间的LSCC骨岛中心透亮影阳性率相比,差异无统计意义(P0.05),在A组的发生率最高,阳性率随年龄增大而减小。结论婴幼儿正常内耳结构的HRCT测量有助于了解内耳结构的发育特点,为CI前评价内耳情况提供参考依据。
[Abstract]:Objective to evaluate the condition of inner ear before cochlear implantation (CI), and to evaluate the development of inner ear by HRCT measurement of normal inner ear structure in infants. Materials and methods 129 patients (258 ears) with normal temporal bone HRCT images were prospectively divided into group A (n = 92) (184 ears), B group (n = 21) 1 ~ 12 months (n = 21), C group (n = 16) (n = 16, n = 32, 25 ~ 36 months). The median height of cochlea, the diameter of basal circumference of cochlea, the inner diameter of basal pericochlea, the (CNC) width of cochlear canal, the long diameter of cochlear aqueduct (CA), the diameter of CA outer mouth and the diameter of (LSCC) island of outer semicircular canal were measured. Results there was no significant difference in cochlear basal circumference diameter, inner diameter of cochlea, height of middle parietal circumference, CNC width and LSCC bone island width in different age, sex and lateral sex (P0.05). The difference was not statistically significant (P0.05), but increased with age. There was no statistical significance between the positive rate of LSCC bone island center transparency in different sex and side (P0.05). The positive rate was the highest in group A, and the positive rate decreased with the increase of age. Conclusion HRCT measurement of normal inner ear structure in infants is helpful to understand the developmental characteristics of inner ear structure and provide reference for evaluating inner ear condition before CI.
