膝关节J形征在复发性髌骨脱位患者中的发病率、分度及放射学特征

【摘要】:目的:探究膝关节J形征在复发性髌骨脱位患者中的发病率、分度及放射学特征。方法:对59例连续的复发性髌骨脱位患者进行回顾性分析,根据患者在清醒状态下膝关节近伸直位时的髌骨外侧位移程度,将J形征分为阴性(-)、一度阳性(轻度,1+)、二度阳性(明显,2+)三组。采用膝关节0°位CT量化髌骨外移和外倾程度,参数包括:髌骨平分指数、髌骨滑车距离、髌骨外倾角。结果:入选的59例患者中:72.9%(43/59)存在J形征阳性,其中27.1%(16/59)一度阳性(1+),45.8%(27/59)二度阳性(2+)。3个CT参数均随J形征分度的增加而增大(P0.05)。结论:在59例连续的复发性髌骨脱位病例中,J形征阳性率为72.9%,J形征的严重程度与髌骨外移和外倾程度呈正相关性。
[Abstract]:Objective: to investigate the incidence, grading and radiologic features of J shape sign of knee joint in patients with recurrent patellar dislocation. Methods: 59 consecutive patients with recurrent patellar dislocation were retrospectively analyzed. According to the degree of lateral displacement of patella in the proximal extension of knee joint in awake state, J sign was classified as negative (-) and once positive (mild, 1). Two degree positive (obvious, 2) three groups. The degree of patellar outward displacement and extroversion was measured by CT at 0 掳position of knee joint. The parameters included patellar equalization index, patellar trochlear distance and patellar external inclination angle. Results: among the 59 patients enrolled, 72.9% (43 / 59) had J positive, of which 27.1% (16 / 59) were once positive (1). 45.8% (27 / 59) second-degree positive (2). The three CT parameters increased with the increase of J-index (P0.05). Conclusion: in 59 consecutive cases of recurrent patellar dislocation, the positive rate of J sign is 72.9%. The severity of J sign is positively correlated with the degree of patella outward migration and extroversion.
