OsiriX测量感染性角膜炎的光学相干断层图像的可行性分析

【摘要】:目的 分析OsiriX用来测量感染性角膜炎在眼前段光学相干断层成像(AS-OCT, anterior segment optical coherence tomography)的可行性,测量指标包括浸润部位角膜厚度,浸润面积以及与浸润部位密度变化相关的值。 资料与方法 回顾性分析2011年12月至2012年3月在我院诊断为感染性角膜炎的病人的32张AS-OCT图片。首先比较Visante OCT机器自带软件所测量的浸润部位中心角膜厚度与OsiriX测量所得的浸润部位中心角膜厚度是否具有一致性。再分析OsiriX测量感染性角膜炎AS-OCT图片上的浸润面积与OCT值的可行性,评估指标包括测量的可重复性和可再现性。Visante OCT与OsiriX两种测量方法的一致性采用配对T检查,Pearson线性相关分析,Bland-Altman分析法综合分析。OsiriX测量感染性角膜炎AS-OCT图片的可行性采用配对T检查和组内相关系数(intraclass correlation coefficients, ICC)综合分析。以P0.05作为差异有统计学差异。 结果 1、Visante OCT与OsiriX测量所测量的浸润部位中心角膜厚度,平均值分别为700.19±171.67μm和694±166.84μm,差值的平均值为6.21±10.45μm,差异具有统计学意义(P0.05)。Pearson线性相关分析示两种方法测量所得的浸润部位中心角膜厚度具有高度的相关性(相关系数r为0.998,P0.05)。由Bland—Altman分析法示仅3.1%(1/32)在95%一致性区间之外,两者的一致性较好。 2、OsiriX测量感染性角膜炎浸润病灶面积,同一操作者两次测量的结果平均值分别是2207641.94±1196023.88μm2和2170665.94±1170061.01“m2,两者结果无显著性差异(P值0.05),具有良好的可重复性(ICC0.9)。两个不同操作者测量结果的平均值分别是2207641.94±1196023.88μm2和2201682.04±1177914.01μm2,P值为0.780.05,结果无显著性差异(P值0.05),具有良好的可再现性(ICC0.9)。 3、OsiriX测量感染性角膜炎浸润病灶OCT值,同一操作者两次测量的结果平均值分别是368.38±135.22和376.72±133.54,结果无显著性差异(P值0.05),具有良好的可重复性(ICC0.9)。两个不同操作者两次测量的结果平均值分别是368.38±135.22和371.63±130.06,结果无显著性差异(P值0.05),具有良好的可再现性(ICC0.9)。 结论 OsiriX测量感染性角膜炎浸润病灶中心部位角膜厚度与Visante OCT机器自带的软件具有良好的一致性,其测量不规则病灶的面积的功能以及和病灶密度相关的OCT值的功能具有较好的可重复性和可再现性,因此认为OsiriX能够用于测量感染性角膜炎AS-OCT的图片。
[Abstract]:Objective to analyze the feasibility of using OsiriX to measure the corneal thickness of infective keratitis in anterior segment optical coherence tomography (AS-OCT, anterior segment optical coherence tomography). The infiltration area and the value related to the change of the density of the infiltrating site. Materials and methods 32 AS-OCT images of patients with infectious keratitis from December 2011 to March 2012 were analyzed retrospectively. The thickness of the central cornea of the infiltrating site measured by Visante OCT software was compared with that of the central corneal thickness of the infiltrating site measured by OsiriX. The feasibility of OsiriX in measuring the area of infiltration and the value of OCT on AS-OCT images of infectious keratitis was analyzed. The evaluation indexes included reproducibility and reproducibility of. Visante OCT and OsiriX. Pearson linear correlation analysis and Bland-Altman analysis were used to analyze the feasibility of OsiriX in the measurement of AS-OCT images of infectious keratitis by paired T test and (intraclass correlation coefficients, ICC) comprehensive analysis of intragroup correlation coefficient. With P0.05 as the difference was statistically significant. Results 1Visante OCT and OsiriX were used to measure the central corneal thickness of the infiltrating site. The mean values were 700.19 卤171.67 渭 m and 694 卤166.84 渭 m, respectively, and the difference was 6.21 卤10.45 渭 m. The difference was statistically significant (P0.05). Pearson linear correlation analysis showed that there was a high correlation between the central corneal thickness measured by the two methods (r = 0.998 P 0.05). Bland-Altman analysis shows that only 3.1% (1 / 32) is outside the 95% consistency range. 2OsiriX was used to measure the area of infectious keratitis. The mean values of the two measurements were 2207641.94 卤1196023.88 渭 m ~ 2 and 2170665.94 卤1170061.01 "m ~ 2, respectively. There was no significant difference between the two results (P < 0.05). It has good repeatability (ICC0.9). The average values of two different operators were 2207641.94 卤1196023.88 渭 m ~ 2 and 2201682.04 卤1177914.01 渭 m ~ 2, P = 0.780.05.The results showed no significant difference (P < 0.05) and had good reproducibility (ICC0.9). 3OsiriX was used to measure the OCT value of infectious keratitis infiltrating foci. The mean values of the two measurements of the same operator were 368.38 卤135.22 and 376.72 卤133.54, respectively. There was no significant difference in the results between the two groups (P < 0. 05). It has good repeatability (ICC0.9). The mean values of the two measurements were 368.38 卤135.22 and 371.63 卤130.06, respectively. There was no significant difference in the results (P 0. 05), and there was good reproducibility (ICC0.9). Conclusion the measurement of corneal thickness at the center of infectious keratitis by OsiriX is in good agreement with the software of Visante OCT machine. The function of measuring the area of irregular lesions and the OCT value related to the density of the lesion is reproducible and reproducible. It is considered that OsiriX can be used to measure the AS-OCT images of infectious keratitis.
