多层螺旋CT血管成像和数字减影血管造影图像质量的研究

【摘要】:目的 本文的通过模体模拟分析数字减影血管造影(DSA)和多层螺旋CT(MSCT)对血管成像对比度、分辨率和密度值等的差异,寻找出不同病人不同造影剂浓度对DSA和CT成像的影响因素,从而根据病人和造影剂浓度的不同选择诊断冠心病的检查方式。 材料与方法 本文采用由丙烯酸树脂平板和水箱自制的水模体,模拟不同密度的造影剂和不同管径的血管,并分别在DSA和MSCT下进行成像扫描,分析两种成像设备的物理性能的差异、二者图像的血管在噪声密度分辨率和密度值的差异。由两名资深影像科医师采用双盲法对两种设备做出的图像进行综合分析,选择能分辨的最小血管影像作为低分辨率对比度影像。 结果 根据不同成像系统的特点,将剂量优化方案分为2组,并用两种成像系统分别对血管体模进行检查。MSCT分别进行高剂量和低剂量扫描,获取一组数据;DSA进行不同透视和电影模式,,获取第二组图像数据。 结论 数字减影血管造影DSA成像质量和照射剂量产生影响,是由系统中的透视条件及摄影条件照射野和附加滤过的大小,影像增强器尺寸、视角的几何放大。都可影响字减影血管造影DSA产生的照射剂量和成像质量;而MSCT系统中的扫描方式、机架转速、探测器宽度、FOV大小等则对CT成像质量和辐射剂量产生影响。高剂量条件下,两种设备做出的两组影像没有明显差异。但在低剂量条件,获得满足影像诊断要求的情况下,MSCT比DSA具有更高的输出剂量。因此,在不影响诊断要求的前提下,选择DSA合理的成像方式,可以在获得高质量影像同时,降低病人所受的辐射剂量。
[Abstract]:Objective to analyze the differences of contrast, resolution and density between digital subtraction angiography (DSA) and multi-slice spiral CT (MSCT) (MSCT) in digital subtraction angiography (DSA) and multi-slice spiral angiography (CT (MSCT). To find out the influencing factors of different contrast agent concentration on DSA and CT imaging in different patients, so as to select the examination method of diagnosing coronary heart disease according to the patient and contrast medium concentration. Materials and methods A water template made of acrylic resin plate and water tank was used to simulate different density contrast agents and vessels with different diameters, and the images were scanned under DSA and MSCT, respectively. The difference of the physical performance between the two imaging devices and the difference of the noise density resolution and density value between the two images are analyzed. A double blind method was used by two senior imaging physicians to analyze the images made by the two kinds of equipment. The smallest vessel image which could be distinguished was selected as the low resolution contrast image. Results according to the characteristics of different imaging systems, the dose optimization scheme was divided into two groups, and the phantom was examined by two kinds of imaging systems. MSCT was scanned with high dose and low dose respectively to obtain a set of data. DSA performs different perspective and movie modes to obtain the second set of image data. Conclusion the effect of DSA imaging quality and irradiation dose on digital subtraction angiography is due to the size of irradiation field and additional filtering, the size of image intensifier and the geometric magnification of the angle of view under the fluoroscopic and photographic conditions in the system. [WT5 "HZ] conclusion [WT5" BZ] [WT5 "BZ] Both of them can affect the irradiation dose and imaging quality of DSA, while the scanning mode, frame rotation speed, detector width and FOV size in MSCT system have influence on the imaging quality and radiation dose of CT. At high dose, there was no significant difference between the two groups of images made by the two devices. However, under the condition of low dose, MSCT has higher output dose than DSA under the condition of satisfying the requirement of imaging diagnosis. Therefore, on the premise that the diagnostic requirements are not affected, the reasonable imaging mode of DSA can obtain high-quality images and reduce the radiation dose of the patients at the same time.
