腹部开放伤合并海水浸泡中内毒素的作用及其机制的实验研究

【摘要】:目的 探讨腹部开放伤合并海水浸泡中内毒素及脂多糖结合蛋白(lipopolysaccharide-binding protein,LBP)和CD14的作用。方法 雄性大鼠60只,随机分为正常组、腹部开放伤对照组、腹部开放伤合并海水浸泡0.5h、1h、2h、3h组(n=10)。检测血浆中的内毒素水平和肝、肺中LBP及CD14蛋白,并进行病理观察。另取雄性大鼠36只,随机分为6组(n=6),检测肝、肺中LBP、CD14及TLR4的mRNA。结果 腹部开放伤合并海水浸泡组各脏器病理变化较重。受伤大鼠经海水浸泡2h后,血浆中内毒素水平明显升高。大鼠肝及肺中LBP及CD14mRNA在浸泡2h后显著升高,而TLR4 mRNA则未见明显变化。肝及肺的LBP、CD14蛋白表达也有类似的趋势。结论 腹部开放伤合并海水浸泡后,内毒素血症上调肝及肺中的LBP、CD14,内毒素-LBP/CD14-炎症介质之间形成“正反馈环”,放大炎症反应,加重损伤。
[Abstract]:Objective to investigate the effect of endotoxin lipopolysaccharide and open abdominal wound with seawater immersion binding protein (lipopolysaccharide-binding protein, LBP) and CD14. Methods: male 60 rats were randomly divided into normal group, open abdominal injury group, 0.5h, open abdominal injury combined with seawater immersion of 1H, 2h, 3h group (n=10) detection of plasma. The level of endotoxin and liver, LBP and CD14 protein in the lung and pathological observation. Another 36 male rats were randomly divided into 6 groups (n=6), the detection of liver, lung LBP, CD14 and TLR4 mRNA. results of the open abdominal injury and seawater immersion group, pathological changes were severe. The injured rats with seawater immersion after 2h, the plasma levels of endotoxin in rats increased significantly. LBP and CD14mRNA in liver and lung was significantly higher in 2H and TLR4 after soaking, mRNA had no obvious change. The liver and lung LBP, the protein expression of CD14 had a similar trend. Conclusion open abdominal injury After seawater immersion, endotoxemia up-regulated LBP and CD14 in liver and lung, and positive and negative feedback loops between endotoxin -LBP/CD14- inflammatory mediators, amplifying inflammatory reaction and aggravating injury.
