四步手法治疗颈性眩晕90例临床观察

【摘要】：目的通过观察四步手法与传统穴位推拿治疗颈性眩晕的疗效比较，探讨四步手法治疗颈性眩晕的临床疗效及作用机理，从而证实四步手法治疗颈性眩晕简单实用、科学规范、疗效确切、安全无副作用。方法将90例入选的颈性眩晕患者，随机分成两组：试验组45人，对照组45人。实验组采用四步手法治疗；对照组采用传统穴位推拿法治疗。分别治疗4个周后，观察2组患者TCD、X线的变化及治疗前后中医临床症状的变化，并对上述结果进行统计学处理。结果 1、观察指标的变化：与治疗前比较，治疗后2组的TCD、X线均有改善，有显著性差异（p0.01）；2组治疗前后中医症状积分比较具有显著性差异（p0.01），四步手法治疗组中医症状积分改善优于传统穴位推拿治疗组。 2、疗效评价：四步手法治疗组与传统穴位推拿治疗组无显著性差异（p0.05），，四步手法治疗组疗效高于传统穴位推拿治疗组（p0.01）；中医症候疗效：四步手法治疗组与传统穴位推拿治疗组有明显差异，差异具有统计学意义(P0.05)，四步手法治疗组疗效高于传统穴位推拿治疗组（p0.01）。 3、安全性评价:本项临床试验研究期间，实验组和对照组均未发现不良反应。结论四步手法能有效缓解颈性眩晕，并能改善患者临床症状，达到改善椎-基底动脉供血，疗效可靠，安全无副作用，四步手法在缓解疼痛方面和传统穴位推拿法无差异，在改善椎-基底动脉供血方面优于传统穴位推拿法，是治疗颈性眩晕的有效方法。
[Abstract]:Purpose The clinical efficacy and mechanism of four-step manipulation in the treatment of cervical vertigo were discussed by observing the curative effect of four-step manipulation and traditional acupoint massage in the treatment of cervical vertigo, which proved that four-step manipulation was simple, practical and scientific in treating cervical vertigo. It is effective and safe without side effects. Method Ninety patients with cervical vertigo were randomly divided into two groups: experimental group (n = 45) and control group (n = 45). The experimental group was treated with four-step manipulation and the control group with traditional acupoint massage. After 4 weeks of treatment, the changes of TCD X ray and the changes of clinical symptoms of TCM before and after treatment were observed, and the above results were analyzed statistically. Result 1. The changes of the indexes: compared with before treatment, the TCD-X ray of the two groups were improved after treatment. There was significant difference in the scores of TCM symptoms before and after treatment. The improvement of TCM symptom score in four-step manipulation group was better than that in the traditional acupoint massage treatment group. 2.Therapeutic evaluation: there was no significant difference between the four-step manipulation treatment group and the traditional acupoint massage treatment group, the curative effect in the four-step manipulation treatment group was higher than that in the traditional acupoint massage treatment group, and the curative effect of TCM syndrome: the four-step manipulation treatment group and the traditional acupoint therapy group. There was significant difference in massage treatment group. The difference was statistically significant (P 0.05). The curative effect of four-step manipulation group was higher than that of traditional acupoint massage group (P 0.01). 3, safety evaluation: during this clinical trial, no adverse reactions were found in the experimental group and the control group. Conclusion The four-step manipulation can effectively relieve cervical vertigo and improve the clinical symptoms of the patients. It can improve the blood supply of vertebrobasilar artery with reliable curative effect and no side effects. There is no difference between four-step manipulation and traditional acupoint massage in relieving pain. It is superior to the traditional acupoint massage in improving the blood supply of vertebrobasilar artery and is an effective method in the treatment of cervical vertigo.
