中西医结合在尘肺治疗中应用探讨

【摘要】：目的根据GBZ 70—2009《尘肺病诊断标准》,探讨各个尘肺期别煤工尘肺(CWP)的中医证候学及证型学特征,为规范中西医结合治疗尘肺提供参考。方法随机选择200例观察对象和320例CWP患者,分析其中医证候、证型特征及中西医结合辨证施治效果。结果观察对象组、CWP壹期组、贰期组和叁期组之间年龄差异有统计学意义(P0.001);观察对象证候表现轻微;CWP壹期表现为干咳、少量黄黏痰、喘息,证型以实证为主;CWP贰期表现为咳嗽、中量黏痰、喘息、神疲乏力、腰膝酸软、纳少等,证型实虚夹杂;CWP叁期表现以喘憋、大量痰液、发绀等,证型以虚证为主(P0.05~0.005)。对CWP进行辨证施治取得明显疗效(P0.05~0.005)。结论随着CWP的进展,证候表现由实到虚,血瘀贯穿始终;《尘肺病诊断标准》将其尘肺分为壹、贰、叁期科学性强,中医如在此基础上辨证论治,有利于治疗规范化。
[Abstract]:Objective according to GBZ 70-2009 diagnostic criteria of pneumoconiosis, to explore the characteristics of TCM syndromes and syndromes of (CWP) in different stage of pneumoconiosis, and to provide reference for regulating the treatment of pneumoconiosis by combining traditional Chinese and western medicine. Methods 200 patients with CWP and 320 patients with CWP were randomly selected and their syndromes and syndromes were analyzed. Results there was significant difference in age between the two groups (P0. 001), the symptoms of CWP were mild, the symptoms of CWP were dry cough, a small amount of yellow sticky phlegm, wheezing, and the syndrome type was mainly positive. In the second stage of CWP, the symptoms were cough, moderate amount of sputum, wheezing, fatigue, weakness of the waist and knee, and deficiency of the syndrome, while in the third stage of CWP, the symptoms were dyspnea, sputum, cyanosis, etc. The syndrome type was mainly deficiency syndrome (P0.050.005). The treatment of CWP based on differentiation of symptoms and signs was effective (P 0.05 0. 005). Conclusion along with the progress of CWP, the syndromes show from reality to deficiency, blood stasis runs through all the time, the diagnosis standard of pneumoconiosis divides its pneumoconiosis into one stage, two stages and three stages, which is scientific, and it is beneficial to the standardization of treatment if Chinese medicine is based on differentiation of syndrome and treatment.
