中西医结合治疗急性早幼粒细胞白血病21例回顾性研究

【摘要】： 目的:回顾性研究应用不同诱导缓解方案及诱导缓解后应用不同巩固维持治疗方案治疗急性早幼粒细胞白血病(APL)的临床疗效,观察应用中医药分期辨证论治治疗APL的临床效果。 方法:回顾性分析辽宁中医药大学附属医院1999年-2007年收治的初治急性早幼粒细胞白血病病例21例,按照不同的诱导缓解治疗方案比较各组的完全缓解率(CR)、达到CR的时间及复发例数和生存时间。 结果:21例初治患者中达到CR19例,死亡2例,总CR率89.5%。采用ATRA单诱导缓解方案治疗的2例中,1例患者死亡,另一例获得CR的时间为42.0d;采用As2O3单诱导缓解方案治疗的10例中,第1疗程达CR者2例,此组CR率为90.0%,获得CR的平均时间为38.3d,死亡1例;采用ATRA+As2O3双诱导缓解治疗的9例中,第1疗程达CR者5例,此组CR率为100%,获得CR的平均时间为30.5d。 结论:对初发的APL患者,ATRA联合As2O3双诱导配合中医药分期辨证论治,可获得较高的完全缓解率,获得完全缓解的时间缩短,且获得长期生存者多,无复发病例。
[Abstract]:Objective: to study the clinical efficacy of different induction remission regimens and different consolidation and maintenance regimens in the treatment of acute promyelocytic leukemia (APL). To observe the clinical effect of TCM syndrome differentiation in the treatment of APL. Methods: 21 cases of acute promyelocytic leukemia treated in affiliated Hospital of Liaoning University of traditional Chinese Medicine from 1999 to 2007 were analyzed retrospectively. the complete remission rate of (CR), in each group was compared according to different induced remission schemes. The time to reach CR, the number of recurrence cases and survival time. Results: of the 21 newly diagnosed patients, CR19 was achieved in 2 cases, and the total CR rate was 89.5%. Of the 2 cases treated with ATRA single induced remission regimen, 1 patient died and the other case obtained CR for 42.0 days. Of the 10 cases treated with As2O3 single induced remission regimen, 2 cases reached CR in the first course of treatment. The CR rate was 90.0%, the average time to obtain CR was 38.3 days, and 1 case died. Of the 9 cases treated with ATRA As2O3 double induction remission, 5 cases reached CR in the first course of treatment. The CR rate in this group was 100%, and the average time to obtain CR was 30.5 days. Conclusion: for patients with primary APL, ATRA combined with As2O3 double induction combined with syndrome differentiation of traditional Chinese medicine can obtain a higher complete remission rate, shorten the time to obtain complete remission, and obtain more long-term survival, no recurrence cases.
