【摘要】：[背景]：恶性肿瘤的侵袭和转移是引起患者死亡的主要原因,而肿瘤侵袭和转移是个多步骤多基因参与的复杂过程,其中细胞黏附、基质溶解和细胞移动是癌细胞侵袭转移过程中的3个关键步骤。近年来研究表明,CD44v6和MMP-9参与了肿瘤的发生发展过程,特别是淋巴结转移以及浸润性生长,同时还发现他们在肿瘤中的表达与肿瘤不良预后可能有着密切的关系。[目的]：本研究目的是检测CD44v6和MMP-9在60例原发性胃癌组织及60例癌旁组织中的表达情况,并收集其临床病例资料,分析其在胃癌组织中的表达与胃癌浸润转移的关系以及二者在胃癌浸润转移过程中的相关性。[方法]：收集郑州大学第一附属医院2013年3月至2014年3月期间收治的病理资料及临床资料完整的胃癌根治手术病人60例,所有病例手术前均未曾接受过放、化疗、中医及免疫治疗。选取胃癌组织标本60例,以及癌旁组织60例,(距癌组织大于5厘米),制作蜡块。采用双重免疫组化染色检测CD44v6与MMP-9在胃癌中及相应癌旁正常组织中的表达,数据分析用卡方检验比较二者在不同组织中的表达,卡方检验检验胃癌组织中CD44v6和MMP-9蛋白质的表达与肿瘤患者临床病理特征之间的关系及二者在胃癌组织中表达的相关性。所有的统计分析采用SPSSv17.0。[结果]：1、在胃癌组织中CD44v6蛋白的阳性表达率是81.67%,显著高于在癌旁组织中的阳性表达率20.00%,两者之间差异具有统计学意义(χ2=53.393 p0.01)2、MMP-9蛋白在胃癌组织中的阳性表达率为73.33%,明显高于其在癌旁组织中的表达率5.00%,两者之间差异有统计学意义(χ2=10.568 p0.01)。3、卡方检验分析结果表明,CD44v6蛋白的表达与肿瘤的浸润深度、TNM分期相关(χ2=9.013和8.104 p0.05)。而与患者的年龄、性别、淋巴结转移数目和肿瘤的大小及肿瘤发生位置无明显相关性(p0.05)4、MMP-9蛋白的表达与与患者性别、年龄、淋巴结转移数目以及肿瘤的大小以及肿瘤的发生位置均无明显相关性(p0.05)；而与肿瘤的浸润深度、TNM分期相关(χ2=9.882和8.304 p0.05)。5、胃癌组织中MMP-9与CD44v6蛋白韵表达具有一定相关性(r=0.036,p0.01),且二者在胃癌中的表达呈正相关。[结论]：一、CD44v6和MMP-9蛋白在胃癌组织中的表达水平均高于其在癌旁正常组织,可能在胃癌的进展中起到一定的作用。二、CD44v6和MMP-9蛋白在胃癌组织中的高表达水平与肿瘤浸润深度及临床TNM分期相关,可能在肿瘤浸润及转移过程中发挥一定作用。三、胃癌组织中CD44v6和MMP-9的表达具有相关性,呈正相关。两者可能在肿瘤浸润及转移过程中发挥协同作用。
[Abstract]:[background]: the invasion and metastasis of malignant tumor is the leading cause of death, and the invasion and metastasis of tumor is a complicated process of many steps involved in many genes, including cell adhesion, cell migration and invasion is the dissolution of 3 key steps in the process of metastasis of cancer cells. Recent studies indicate that CD44v6 and MMP-9 in the process of tumor development, especially lymph node metastasis and invasive growth, also may be closely related to them. In the cancer and tumor prognosis: the purpose of this study was to detect the expression of CD44v6 and MMP-9 in 60 cases of primary gastric carcinoma and 60 cases of adjacent in the organization, and collect the clinical data, analysis of the relationship between its expression in gastric cancer tissue and gastric cancer invasion and metastasis as well as the two invasion. In the process of transfer correlation method] in gastric cancer: the collection of Zhengzhou Pathology of the First Affiliated Hospital of University from March 2013 to March 2014 and complete clinical data of 60 cases of gastric cancer patients, all patients were not given radiotherapy, chemotherapy, and immunotherapy of gastric cancer. Expression by double immunohistochemical staining of CD44v6 and MMP-9 in gastric carcinoma and adjacent normal tissues, data analysis using chi square test to compare the two expression in different tissues, the relationship between the chi square test of CD44v6 in gastric cancer tissues and the expression of MMP-9 protein and tumor patients pathological features and two expression in gastric cancer tissues by SPSSv17.0.[]:1.
