非小细胞肺癌患者血清基质金属蛋白酶-9(MMP-9)与Toll样受体4(TLR4)的表达及临床意义

【摘要】:目的:通过检测NSCLC患者中血清MMP-9、TLR4的表达水平,探讨血清TLR4、MMP-9在NSCLC转移过程中的相关性,分析它们在诊断效能及及临床预后判断价值。方法:选自2015年9月1日至2016年1月1日就诊于南昌大学第一附属医院呼吸内科的34例非小细胞肺癌患者为实验组(NSCLC组),同一时期的门诊健康体检者15例为正常对照组,用酶联免疫分析方法(ELISA)分别检测各血清中MMP-9、TLR4的表达水平。结果:1、NSCLC组MMP-9、TLR4的表达水平均较正常对照组高,具有统计学意义(P0.05)。2、NSCLC伴远处转移的患者血清MMP-9、TLR4高于无远处转移的NSCLC患者;晚期(III/IV) NSCLC患者血清MMP-9、TLR4高于早期(Ⅰ/Ⅱ)NSCLC患者,统计学分析示均有P0.05。3、线性回归分析NSCLC患者血清中MMP-9、TLR4的相关性,结果示R=0.939,P0.001,有统计学意义。4、血清MMP-9、TLR4在诊断NSCLC的ROC的AUC分别为0.776、0.702,两者的灵敏度和特异度分别为67.56%和80.00%、80%和50%。5、NSCLC患者血清中MMP-9、TLR4的表达水平与远处转移及临床TNM分期呈相关性,均有P0.05,与年龄、性别、吸烟、病理类型等无明显相关性,统计学分析示(P0.05)。结论:1、NSCLC患者与正常人群相比,其血清MMP-9、TLR4的表达水平显著升高。2、在NSCLC中,伴远处转移的患者与无远处转移的相比,血清MMP-9、 TLR4显著升高;晚期(III/IV)与早期(Ⅰ/Ⅱ)相比,两血清指标显著升高。3、NSCLC患者血清MMP-9、TLR4表达水平呈显著正相关性。4、血清MMP-9、TLR4可在一定程度上作为诊断NSCLC及判断疾病预后的生化指标。
[Abstract]:Objective: to investigate the correlation between serum MMP-9 and MMP-9 in the process of NSCLC metastasis by detecting the expression of MMP-9 in serum of patients with NSCLC. Methods: to analyze their diagnostic efficacy and clinical prognostic value. From September 1st 2015 to January 1st 2016, 34 patients with non-small cell lung cancer (NSCLC) in Department of Respiratory Medicine, first affiliated Hospital of Nanchang University were selected as experimental group (. NSCLC group. The expression of MMP-9 and TLR4 in serum was detected by Elisa in 15 healthy persons in the same period as the normal control group. Results: the expression of MMP-9 and TLR4 in each serum was detected by Elisa. The expression of MMP-9 and TLR4 in NSCLC group was significantly higher than that in normal control group, and had statistical significance in serum MMP-9 of patients with distant metastasis. TLR4 was higher in NSCLC patients without distant metastasis. The level of MMP-9 / TLR4 in patients with advanced stage II / IV NSCLC was higher than that in early stage (P 0.05.3). Linear regression analysis showed the correlation between MMP-9 and TLR4 in serum of patients with NSCLC. The results showed that the serum levels of MMP-9 and TLR4 in patients with NSCLC were significantly higher than those in control group (P 0.001). The AUC of TLR4 in the diagnosis of ROC in NSCLC was 0.776 and 0.702, and the sensitivity and specificity of the two were 67.56% and 80.00%, respectively. The expression of MMP-9 / TLR4 in serum of 80% and 50% patients with NSCLC was correlated with distant metastasis and clinical TNM stage (P0.05, age and sex). There was no significant correlation between smoking and pathological types. Statistical analysis showed that there was no significant correlation between the two groups. Conclusion compared with the normal controls, the serum MMP-9 of the patients with 1 / 1 NSCLC is higher than that of the normal controls. The expression of TLR4 was significantly increased in NSCLC patients with distant metastasis compared with those without distant metastasis.
