MR扩散加权成像与不同成像序列组合对乳腺良恶性病变的诊断价值

【摘要】:目的:探讨MR扩散加权成像与不同成像序列联合应用对乳腺良恶性病变定性诊断价值。方法:回顾性分析71例(良性42例46个病灶,恶性29例32个病灶)经手术及病理证实乳腺良恶性病变患者的MRI资料,以病理诊断为金标准,统计并比较T_1WI结合DWI(T_1WI+DWI)、T_2WI结合DWI(T_2WI+DWI)、DCE、DCE结合DWI(DCE+DWI)这4种诊断方式对乳腺良恶性病变诊断的敏感性、特异性及准确性。结果:T_1WI+DWI、T_2WI+DWI、DCE、DCE+DWI这4种诊断方式对乳腺良恶性病变诊断的敏感性分别为81.3%、90.6%、93.7%、96.8%,特异性分别为73.9%、91.3%、89.1%、93.5%,准确性分别为76.9%、91%、91%、94.9%。结论:DCE联合DWI对诊断乳腺良恶性病变有较高的敏感性、特异性及准确性,对乳腺良恶性病变的诊断与鉴别诊断具有很高的临床价值,是最佳诊断组合序列。非增强组合序列对乳腺良恶性病变的诊断有一定优势,可作为乳腺疾病筛查的MRI技术。
[Abstract]:Objective: to evaluate the value of Mr diffusion weighted imaging combined with different imaging sequences in the qualitative diagnosis of benign and malignant breast lesions. 29 cases of malignant lesions (32 lesions) proved by surgery and pathology of benign and malignant breast lesions of MRI data, pathological diagnosis as the gold standard, The sensitivity of T1WI combined with DWI(T_1WI DWI / T 2WI combined with DWI(T_2WI DWI (DCE + DWI(DCE DWI) in the diagnosis of benign and malignant breast lesions was statistically analyzed and compared. Results the sensitivity of the four diagnostic methods to the diagnosis of benign and malignant breast lesions was 81.3% and 90.6% and 93.796.8 respectively. The specificity was 83.9% and the specificity was 83.9%. The specificity was 83.9%, and the accuracy was 76.911%. Conclusion the diagnostic sensitivity of the two methods is higher than that of the control group. Non-enhanced combination sequence has some advantages in the diagnosis of benign and malignant breast lesions. It can be used as MRI technique for breast disease screening.
