口腔潜在恶性病变中肿瘤干细胞标记物CD44与CD133表达研究及其与临床因素相关性分析

【摘要】:目的:探究肿瘤干细胞标记物CD44、CD133在口腔正常黏膜上皮、口腔潜在恶性病变和口腔鳞癌组织中的表达及相互关系,以及二者在人口腔黏膜异常增生细胞株DOK细胞株及人口腔鳞癌细胞株CAL-27细胞株中的表达,同时分析CD44、CD133表达与患者临床因素之间的关系;探究CD44和CD133在口腔潜在恶性病变发生和癌变过程中的表达规律及其可能机制,借此评价二者作为口腔潜在恶性病变癌变监测指标、口腔鳞癌早期检测指标的临床价值。方法:采用免疫组织化学双重染色技术检测CD44、CD133在口腔正常黏膜、口腔扁平苔藓、口腔白斑、口腔鳞癌组织中的表达水平。引进并培养DOK细胞株和CAL-27细胞株,采用细胞免疫荧光双重染色技术观察两细胞株中CD44、CD133共表达情况;采用细胞免疫化学技术检测两细胞株中CD44、CD133的表达含量;采用蛋白免疫印迹技术检测两细胞株中CD44、CD133的蛋白含量。收集青岛大学医学院附属医院口腔科和病理科就诊并随访存档的口腔黏膜潜在恶性病变及口腔鳞癌的存档病例,分析评价患者各项临床资料与CD44、CD133表达相关性。采用SPSS19.0软件,比较口腔正常黏膜、口腔扁平苔藓、口腔白斑、口腔鳞癌组织中CD44、CD133的表达情况。比较DOK细胞株和CAL-27细胞株中CD44、CD133的含量及变化。分析CD44、CD133表达与患者临床因素之间的关系。探究CD44及CD133在口腔黏膜癌变过程中的表达规律及其可能机制,借此评价二者作为口腔潜在恶性病变癌变监测指标、口腔鳞癌早期检测指标的临床价值。结果:口腔正常黏膜、口腔潜在恶性病变、口腔鳞癌三组中,CD44的表达率分别为100.00%(10/10)、96.67%(58/60)、71.67%(43/60),表达逐渐减弱(P0.05);CD133的表达率分别为0.00%(0/10)、35.00%(21/60)、63.33%(38/60),表达逐渐增强(P0.05);CD44与CD133的表达有一定相关性(P0.05)。DOK细胞株和CAL-27细胞株中存在CD44和CD133共表达。与DOK细胞株相比,CAL-27细胞株中CD44表达减弱,CD133表达增强。CD44、CD133表达与临床因素相关性分析中,两者表达在患者的年龄、性别、吸烟史、饮酒史及病变部位分组中的差异均无统计学意义(P0.05)。CD44、CD133表达在口腔鳞癌患者的各临床分期、淋巴结转移中的差异有统计学意义(P0.05)。结论:在口腔潜在恶性病变癌变发生和发展的过程中,CD44表达逐渐下降,CD133表达逐渐增高,且CD44和CD133两者表达存在一定相关性,同时二者表达与口腔鳞癌的临床分期和淋巴结转移有关。CD44介导的细胞黏附与分化,CD133维持的干细胞特性可能在口腔潜在恶性病变癌变发生和发展的过程中发挥了重要作用。CD44和CD133作为口腔潜在恶性病变癌变监测指标、口腔鳞癌早期检测指标可能具有重要的临床价值。
[Abstract]:Objective: to investigate the expression and relationship of tumor stem cell marker CD44hCD133 in normal oral mucosa epithelium, oral potential malignant lesions and oral squamous cell carcinoma.The expression of CD4pCD133 in human oral mucosal dysplasia cell line DOK cell line and human oral squamous cell carcinoma cell line CAL-27 cell line was analyzed, and the relationship between CD4pCD133 expression and clinical factors was analyzed.To explore the expression and possible mechanism of CD44 and CD133 in the process of occurrence and carcinogenesis of oral potential malignant lesions, and to evaluate their clinical value as a monitoring index for carcinogenesis of oral potential malignant lesions and early detection indexes for oral squamous cell carcinoma (OSCC).Methods: Immunohistochemical double staining technique was used to detect the expression level of CD44 and CD133 in normal oral mucosa, oral lichen planus, oral leukoplakia and oral squamous cell carcinoma.DOK cell line and CAL-27 cell line were introduced and cultured. The co-expression of CD44hCD133 in the two cell lines was observed by double immunofluorescence staining, and the expression of CD44hCD133 in the two cell lines was detected by cytoimmunochemical technique.The protein content of CD44 and CD133 in two cell lines was detected by Western blot.Cases of potential malignant lesions of oral mucosa and oral squamous cell carcinoma (SCC) were collected from Department of Stomatology and Department of Pathology of affiliated Hospital of Medical College of Qingdao University.SPSS19.0 software was used to compare the expression of CD44 and CD133 in normal oral mucosa, oral lichen planus, oral leukoplakia and oral squamous cell carcinoma.The contents and changes of CD44 and CD133 in DOK and CAL-27 cell lines were compared.To analyze the relationship between the expression of CD 44 and CD 133 and the clinical factors.To explore the expression and possible mechanism of CD44 and CD133 in the process of oral mucosa carcinogenesis, and to evaluate their clinical value as a monitoring index for the carcinogenesis of oral potential malignant lesions and early detection indexes for oral squamous cell carcinoma (OSCC).Results: normal oral mucosa, potential malignant lesions of oral cavity,鍙h厰槌炵檶涓夌粍涓,
