CT引导下穿刺活检对骨肿瘤病变诊断的临床应用价值

【摘要】：目的:探讨CT引导下穿刺活检对骨肿瘤病变诊断的临床应用价值。方法:回顾性分析于2016年10月至2017年2月在本医疗组出院曾行CT引导下穿刺活检的52例患者,男26例,女26例,年龄7-78岁,平均年龄47.2岁;对比穿刺病理结果与临床诊断最终结果;分析其穿刺活检病理阳性、阴性结果及活检正确率来评价CT引导下穿刺活检对骨骼病变诊断的临床应用价值。结果:所有52例患者共行55例穿刺活检,取材成功率100%,其中获得明确诊断50例(92.7%)。52例患者活检总准确率为96.2%,其中34例患者结合病史和临床血生化指标,33证实符合诊断,1例为假阴性;余18例患者经外科手术,17获得病理证实,1例经病理提示为假阴性。并发症方面,所有患者穿刺活检后未发现大量出血、神经损伤等严重并发症。结论:CT引导下穿刺活检对骨骼病变的诊断准确率高,且并发症少,是明确诊断骨骼病变的较好选择。
[Abstract]:Objective: to evaluate the value of CT-guided biopsy in the diagnosis of bone tumor.Methods: from October 2016 to February 2017, 52 patients (26 males and 26 females, aged 7-78 years, with an average age of 47.2 years) who had undergone CT-guided biopsy were retrospectively analyzed.To evaluate the clinical value of CT-guided biopsy in the diagnosis of bone diseases, the positive and negative results of biopsy and the correct rate of biopsy were analyzed by comparing the results of biopsy pathology with the final results of clinical diagnosis.Results: 55 biopsies were performed in all 52 patients.The total accuracy of biopsy was 96.22in 50 cases with definite diagnosis of 92.70.52 cases, of which 34 cases were confirmed to be false-negative in accordance with the history and clinical biochemical indexes (P < 0.05).The remaining 18 cases were confirmed pathologically by surgery and 1 case was false-negative.In terms of complications, no severe complications such as massive bleeding and nerve injury were found in all patients after biopsy.Conclusion the accuracy of diagnosis of bone lesions is high, and the complications are few, so it is a better choice for definite diagnosis of bone diseases.
