NANOG与CTLA-4在加权Youden指数的合并效应量估计方法下的系统分析

【摘要】:本论文是通过收集已有的关于NANOG同CTLA-4蛋白在临床试验的研究数据,并用加权Youden指数合并效应量的方法来改进统计软件使用的模型,最终通过改进的方法模型去获得NANOG同CTLA-4与胃癌肿瘤细胞之间的关系以及分析出药物Ipilimumab在治疗黑色素瘤过程中相对于传统的手术化疗法的效果强弱性。在对于方法改进中首先假设各研究间相互独立,同一个研究中的特异度和灵敏度也相互独立并且服从二项分布。在固定效应模型下建立加权Youden指数合并效应量估计方法并采用最大似然估计法和最小二乘法进行估计并进行比较。在随机效应模型下构建加权Youden指数的合并效应量的估计方法并采用加权最小二乘方法进行估计。最终采用Monte Carlo模拟比较各种方法的统计学性能。数据的获取和分析是通过在PubMed、Mediun、中国生物医学文献数据库(CBM)、知网、维普、万方数据库采用相同的检索策略(关键词:Nanog、CTLA-4、癌症、随机、对照)收集所有Nanog和CTLA-4相关的诊断性试验,对最新的期刊进行手工检索;结合两种检索结果进行资料的提取并应用QUADAS质量评价标准进行质量评估后,采用RevMan分析软件用改进的方法进行系统性分析,得出我们想要的结论。最终的结果表明,在加权Youden指数合并效应量的方法改进下,统计性能有所提高。通过新模型对于数据的统计分析,得出NANOG同胃癌细胞、不同的胃癌分期以及胃癌细胞的转移有着强关联性,并且分析了CTLA-4四组等位基因同胃癌是否有关,最后分析出Ipilimumab相对传统的手术化疗法对于黑色素瘤的治疗有着更加明显的疗效。
[Abstract]:This paper is to improve the model of statistical software by collecting the existing research data about NANOG and CTLA-4 protein in clinical trials and using weighted Youden index to combine effect quantity.Finally, the relationship between NANOG and CTLA-4 and gastric cancer tumor cells was obtained by the improved method model, and the effect of drug Ipilimumab in the treatment of melanoma was compared with that of traditional surgical therapy.In the improvement of the method, it is assumed that each study is independent of each other, the specificity and sensitivity of the same study are independent and the binomial distribution is obtained.Under the fixed effect model, the weighted Youden exponent combined effect quantity estimation method is established, and the maximum likelihood estimation method and the least square method are used to estimate and compare.Under the stochastic effect model, the method of estimating the combined effect quantity of weighted Youden exponent is constructed and the weighted least square method is used to estimate it.Finally, Monte Carlo is used to simulate and compare the statistical performance of various methods.The data were obtained and analyzed by collecting all diagnostic trials related to Nanog and CTLA-4 using the same retrieval strategy (keyword: Nanog CTLA-4, cancer, randomized, control) at PubMeden Mediun, the Chinese Biomedical Literature Database (CBM), Zhiwang, Wiper, and Wanfang.This paper carries on the manual retrieval to the newest periodical, combines two kinds of retrieval results to carry on the data extraction and the application QUADAS quality evaluation standard to carry on the quality appraisal, uses the RevMan analysis software to carry on the systematic analysis with the improved method, obtains the conclusion which we want.The final results show that the statistical performance is improved with the improvement of weighted Youden exponent combination effect quantity.Through the statistical analysis of the new model, it is concluded that there is a strong correlation between NANOG and gastric cancer cells, different stages of gastric cancer and metastasis of gastric cancer cells, and whether the four groups of CTLA-4 alleles are related to gastric cancer.Finally, it is concluded that Ipilimumab is more effective than conventional surgical therapy in the treatment of melanoma.
