DEPDC7在宣威NSCLC中的表达及临床相关性研究

【摘要】:[目的]肺癌的发病率及死亡率占所有恶性肿瘤之首。云南宣威地区肺癌发病率和死亡率均居全国首位。研究宣威地区肺癌独特的发病机制与特异的内在驱动基因可为其治疗和预防提供新的干预靶点。前期研究中,我们采用AgilentmRNA芯片发现DEPDC7 (DEP domain-containing protein7)在宣威和非宣威非小细胞肺癌中存在差异表达。本课题旨在组织蛋白水平初步探讨DEPDC7在宣威和非宣威地区非小细胞肺癌患者的癌和癌旁正常肺组织及各临床病理参数之间的相关性,初步阐述DEPDC7在宣威非小细胞肺癌中可能存在的临床应用价值。[方法]收集2009年1月至2015年12月在昆明医科大学第三附属医院(云南省肿瘤医院)行根治性切除术的165例非小细胞肺癌患者,其中宣威地区63例,非宣威地区102例。通过制作石蜡包埋和病理组织切片,采用免疫组织化学的方法检测DEPDC7在宣威、非宣威地区非小细胞肺癌和癌旁正常组织中的表达部位及强度,并统计其表达情况与各临床病理参数之间的相关性。[结果]1. DEPDC7在非小细胞肺癌和正常肺组织中均有表达,其主要表达于细胞质和细胞核,少量表达于线粒体。2. DEPDC7在非小细胞肺癌组织中表达显著高于癌旁正常组织,差异具有明显统计学意义(68.5% VS 41.8% P0.05)3.分层分析发现DEPDC7在肺腺癌中表达高于肺鳞癌(75.2%VS50%P0.05),低年龄组表达明显高于高年龄组(68.1% VS 56.9%P0.05),在Ⅰ期患者阳性表达率高于Ⅱ+ⅢA(87.5%VS 77.1%P0.05),差异均具有统计学意义。4. DEPDC7在宣威非小细胞肺癌组织中表达显著高于非宣威非小细胞肺癌组织,差异具有统计学意义(85.7% VS 57.8% P0.05)5. DEPDC7在宣威女性肺腺癌组织中表达显著高于非宣威女性腺癌组织,差异具有统计学意义(92%VS61.1% p0.05)[结论]1. DEPDC7在非小细胞肺癌组织中高表达,提示DEPDC7可能与非小细胞肺癌发生有关,尤其是在肺腺癌的发生中可能起重要作用。在临床Ⅰ期患者中高表达,提示其可能参与非小细胞肺癌早期发生,因此,DEPDC7极有可能成为非小细胞肺癌早诊的分子标记。2. DEPDC7在宣威非小细胞肺癌癌组织中显著高表达,提示DEPDC7可能是宣威地区非小细胞肺癌的新候选癌基因,尤其是在宣威女性肺腺癌的发生发展中可能起重要作用。
[Abstract]:Objective: the incidence and mortality of lung cancer is the highest among all malignant tumors. The morbidity and mortality of lung cancer in Xuanwei area of Yunnan Province ranked first in China. The study of the unique pathogenesis and specific intrinsic driving gene of lung cancer in Xuanwei area may provide a new intervention target for its treatment and prevention. In previous studies, AgilentmRNA microarray was used to detect the differential expression of DEPDC7 domain-containing protein 7 in Xuan Wei and non Xuan Wei non-small cell lung cancer (NSCLC). The aim of this study was to explore the correlation of DEPDC7 in cancer and adjacent normal lung tissues of patients with non-small cell lung cancer (NSCLC) in Xuanwei and non-Xuanwei areas and the clinicopathological parameters. The clinical application value of DEPDC7 in Xuanwei non-small cell lung cancer was discussed. [methods] from January 2009 to December 2015, 165 patients with non-small cell lung cancer (NSCLC) underwent radical resection in the third affiliated Hospital of Kunming Medical University (Yunnan Cancer Hospital), including 63 cases in Xuanwei area and 102 cases in non-Xuanwei area. The expression and intensity of DEPDC7 in non-small cell lung cancer (NSCLC) and adjacent normal tissues in Xuanwei and non-Xuanwei were detected by immunohistochemical method by making paraffin-embedded and pathological tissue sections. The correlation between the expression and the clinicopathological parameters was analyzed. [result] 1. DEPDC7 was expressed in both non-small cell lung cancer (NSCLC) and normal lung tissues, mainly in cytoplasm and nucleus, and a little in mitochondria. The expression of DEPDC7 in non-small cell lung cancer tissues was significantly higher than that in adjacent normal tissues, and the difference was statistically significant (68.5% vs 41.8%, P 0.05%, P 0.05). Stratified analysis showed that the expression of DEPDC7 in lung adenocarcinoma was higher than that in lung squamous cell carcinoma (VS50), the expression of DEPDC7 in low age group was significantly higher than that in high age group (68.1% vs 56.9%), and the positive rate in stage 鈪,
