甲状腺微小乳头状癌的颈中央区淋巴结转移的影响因素分析

【摘要】：目的探讨影响甲状腺微小乳头状癌颈中央区淋巴结转移的相关影响因素,并指导临床手术治疗。方法:对新疆医科大学第一附属医院血管甲状腺外科2012年12月至2016年4月于我院首次诊治的360例诊断为甲状腺微小乳头状癌患者的临床病理资料进行LOGIST回顾性分析。结果:360例甲状腺微小乳头状癌(papillary thyroid microcarcinoma,PTMC)患者中,共有103例(28.61%)有淋巴结转移:其中78例(21.67%)仅有中央区淋巴结转移,19例(5.28%)有中央区淋巴结+颈侧区淋巴结转移,6例(1.67%)仅有颈侧区淋巴结转移。患者颈中央区淋巴结转移与性别、年龄、癌灶直径明显相关(P0.05)。结论:结合患者性别、年龄、癌灶直径等因素,在手术医师经验丰富、技术纯熟的前提下,建议为PTMC患者行预防性颈中央区淋巴结清扫(central lymph node dissection,CLND)。
[Abstract]:Objective to investigate the influencing factors of cervical lymph node metastasis in thyroid micropapillary carcinoma. Methods: the clinical and pathological data of 360 patients diagnosed as thyroid micropapillary carcinoma from December 2012 to April 2016 in the first affiliated Hospital of Xinjiang Medical University were retrospectively analyzed by LOGIST. Results among the 360 patients with (papillary thyroid microcarcinoma of thyroid micropapillary carcinoma, There were 103 cases (28.61%) with lymph node metastasis, of which 78 cases (21.67%) had only central lymph node metastasis, 19 cases (5.28%) had central lymph node metastasis, and 6 cases (1.67%) had cervical lateral lymph node metastasis. Lymph node metastasis in the central cervical region was significantly correlated with sex, age and tumor diameter (P0.05). Conclusion: combined with sex, age and diameter of cancer focus, it is suggested that (central lymph node dissectionCLND should be performed in patients with PTMCs on the premise that the surgeon is experienced and skillful.
