上皮-间质转化调控蛋白在前列腺癌侵袭、转移中的作用及其临床预后诊断价值

【摘要】：目的探讨上皮-间质转化(EMT)调控蛋白在前列腺癌(PCa)侵袭、转移中的作用及其对临床预后的诊断价值。方法收集48例PCa组织样本(PCa组)和50例良性前列腺增生(BPH)组织样本(BPH组),免疫组化SP法检测样本神经钙黏素(N-cadherin)、上皮钙黏素(E-cadherin)表达,分析N-cadherin、E-cadherin表达与PCa患者临床资料的关系。对PCa患者进行随访,记录患者总生存期(OS);以OS作为评价指标,采用单变量和多变量Cox比例风险模型评价患者预后的影响因素。以转化生长因子β1(TGF-β1)诱导DU-145细胞发生EMT,细胞增殖实验检测细胞增殖能力,Transwell实验检测细胞迁移能力,Western blot检测细胞E-cadherin、N-cadherin蛋白表达。结果 PCa组N-cadherin表达水平显著高于BPH组,E-cadherin表达水平显著低于BPH组(均P0.05)。肿瘤直径≥2.5cm者N-cadherin高表达率显著高于肿瘤直径2.5cm者,Ⅳ期PCa患者N-cadherin高表达率显著高于Ⅱ期和Ⅲ期PCa患者,淋巴结转移者N-cadherin高表达率显著高于无淋巴结转移者,远处转移者N-cadherin高表达率显著高于无远处转移者(均P0.05)。Gleason分级≥7分者E-cadherin高表达率显著低于Gleason分级≤6分者,有淋巴结转移者E-cadherin高表达率显著低于无淋巴结转移者,有远处转移者E-cadherin高表达率显著低于无远处转移者(均P0.05)。N-cadherin高表达患者OS显著低于N-cadherin低表达患者(P=0.024),E-cadherin高表达患者OS显著高于E-cadherin低表达患者(P=0.017)。单因素和多因素分析显示,T4期肿瘤、N-cadherin高表达、E-cadherin低表达是影响患者OS的独立危险因素(均P0.05)。细胞增殖实验显示,第24~96h,DU-145组吸光度值显著高于NC组(均P0.05);Transwell实验显示,DU-145组穿膜细胞数量显著多于NC组(P0.05);Western blot实验显示,DU-145组N-cadherin蛋白表达水平显著高于NC组,E-cadherin蛋白表达水平显著低于NC组(均P0.05)。结论 EMT与PCa的增殖、侵袭和迁移有关,N-cadherin、E-cadherin有可能作为PCa临床预后的诊断指标之一。

Methods the expression of N-cadherin (N-cadherin) and E-cadherin (E-cadherin) in 48 patients with PCA and 50 patients with benign prostatic hyperplasia (BPH) were detected by immunohistochemical SP method. The relationship between the expression of N-cadherin and the clinical data of PCA was analyzed. Patients with PCA were followed up to record the total survival time (OS), and the single and multivariate Cox proportional risk models were used to evaluate the prognostic factors. Transforming growth factor 尾 1 (TGF- 尾 1) was used to induce EMTs in DU-145 cells. Cell proliferation assay and transwell assay were used to detect cell migration ability. Western blot was used to detect E-cadherin N-cadherin protein expression in DU-145 cells. Results the expression of N-cadherin in PCA group was significantly higher than that in BPH group (P0.05).
