临床超声及细胞病理学特征对甲状腺微小乳头状癌中央区淋巴结转移的预测价值

【摘要】：目的探讨临床、超声、细胞病理学特征对甲状腺微小乳头状癌(PTMC)中央区淋巴结转移(CLNM)的预测价值。方法收集110例经术后组织病理证实为PTMC并行中央区淋巴结清扫的病例,回顾性分析PTMC组织病理学特征,及术前临床、超声、细胞病理学特征与CLNM的相关性。结果纳入的110例PTMC病例中,共31例(28.2%)出现CLNM。PTMC的病理特征中,病灶5mm、多灶性、多侧性、甲状腺包膜侵犯、侧区淋巴结转移与CLNM显著相关(P0.05)。术前特征中,患者年龄≤38岁、超声测量可疑病灶6mm、可疑多灶性、多侧性及可疑淋巴结转移或包膜侵犯等为CLNM的危险因素(P0.05)。结论术前通过分析可疑PTMC患者临床、超声资料,对CLNM具有一定预测价值。

Methods the histopathological features of PTMC with central lymph node dissection were retrospectively analyzed in 110 patients with PTMC confirmed by histopathology, and the correlation between preoperative clinical, ultrasound, cytopathological features and CLNM was analyzed retrospectively. Results among the 110 cases of PTMC, 31 (28.2%) had the pathological features of CLNM.PTMC.
