胃癌及其癌前病变中幽门螺杆菌感染和c-Jun及ATF-2的表达

【摘要】:目的观察胃癌及其癌前病变中幽门螺旋杆菌(Helicobacter pylori,Hp)的感染和c-Jun、ATF-2蛋白表达情况,探讨胃癌发生发展机制。方法采用免疫组织化学染色技术检测浅表性胃炎(20例)、肠化(20例)、不典型增生(28例)、早期胃癌(36例)、进展期胃癌(73例)中幽门螺杆菌(HP)感染和c-Jun、ATF-2蛋白的表达情况,用半定量评分方法评价各组c-Jun、ATF-2表达,通过统计学分析探究其相互关系。结果免疫组化染色发现,每组均有程度不同的Hp表达,主要存在于胃小凹或者胃黏膜上皮细胞内,早期胃癌组及进展期胃癌组Hp主要存在于癌灶周围正常黏膜组织内,癌灶内没有Hp的表达;统计学分析可见肠化组(30.00%)和不典型增生组(39.29%)Hp检出率均明显高于浅表性胃炎组(5.00%)、早期胃癌组(2.78%)和进展期胃癌组(1.39%),差异有显著性意义(P0.05)。c-Jun染色定位于细胞核,在浅表性胃炎组、肠化组可见少量c-Jun表达,早期胃癌及进展期胃癌组c-Jun表达率相对较高,早期胃癌组c-Jun表达率最高;不典型增生组(21.43%)、早期胃癌组(52.78%)、进展期胃癌组(45.21%)c-Jun表达率均高于浅表性胃炎组(10.00%)(P0.05),早期胃癌组c-Jun表达率高于进展期胃癌组(P0.05),胃癌组(47.71%)c-Jun表达率明显高于非胃癌组(16.18%)(P0.05)。ATF-2在浅表性胃炎组未见阳性,其余4组均有多少不等的ATF-2表达,染色均定位于细胞核;肠化组(10.00%)、不典型增生组ATF-2低表达,早期胃癌组(38.90%)、进展期胃癌组(41.10%)ATF-2高表达,胃癌组(40.37%)ATF-2表达率高于非胃癌组(5.88%)(P0.05)。进展期胃癌组中1例Hp感染阳性的病人c-Jun、ATF-2表达均显示阳性。结论Hp感染与胃粘膜上皮肠上皮化生及不典型增生有关;胃癌的形成过程中涉及c-Jun、ATF-2的高表达。
[Abstract]:Objective to observe the infection of Helicobacter pylori (Helicobacter pylori) HP and the expression of c-Junjiao ATF-2 protein in gastric cancer and its precancerous lesions, and to explore the mechanism of gastric cancer. Methods Immunohistochemical staining was used to detect the infection of Helicobacter pylori (HP) and the expression of c-Jun-ATF-2 protein in superficial gastritis (20 cases), intestinal metaplasia (20 cases), atypical hyperplasia (28 cases), early gastric cancer (36 cases) and advanced gastric cancer (73 cases). The expression of c Junan ATF-2 was evaluated by semi-quantitative scoring method and its relationship was studied by statistical analysis. Results Immunohistochemical staining showed that there were different levels of HP expression in each group, mainly in gastric fovea or gastric mucosal epithelium, while in early gastric cancer and advanced gastric cancer, HP mainly existed in the normal mucosa around the tumor. The positive rate of HP in intestinal metaplasia group (30.00%) and atypical hyperplasia group (39.29%) was significantly higher than that in superficial gastritis group (5.00%), early gastric cancer group (2.78%) and advanced gastric cancer group (1.39%). In superficial gastritis group, a small amount of c-Jun expression was found in intestinal metaplasia group. The expression rate of c-Jun was relatively high in early gastric cancer and advanced gastric cancer, and the highest in early gastric cancer group. The expression rate of c-Jun in atypical hyperplasia group (21.43%), early gastric cancer group (52.78%), advanced gastric cancer group (45.21%) was higher than that in superficial gastritis group (10.00%) (P0.05). The c-Jun expression rate in early gastric cancer group was higher than that in advanced gastric cancer group (P0.05), and the c-Jun expression rate in gastric cancer group (47.71%) was significantly higher than that in non-gastric cancer group (16.18%) (P0.05). No positive results were found in patients with sexual gastritis. The expression of ATF-2 in intestinal metaplasia group (10.00%), atypical hyperplasia group (38.90%), advanced gastric cancer group (41.10%), ATF-2 expression rate in gastric cancer group (40.37%) was higher than that in non-gastric cancer group (5.88%) (P0.05). In the advanced gastric cancer group, one patient with HP infection showed positive expression of c-Junjiao ATF-2. Conclusion HP infection is associated with intestinal metaplasia and atypical hyperplasia of gastric mucosa epithelium, and the high expression of c-Junan ATF-2 is involved in the formation of gastric carcinoma.
