cIAP和Caspase-7在子宫内膜腺癌中的表达及意义

【摘要】:目的:研究凋亡抑制蛋白cIAP和凋亡蛋白Caspase-7分别在不同子宫内膜组织中的表达,分析二者的表达与子宫内膜腺癌组织临床分期(FIGO 2009)、病理分级、淋巴结转移等临床生物学特征的关系,及二者表达的相关性,探讨二者在子宫内膜组织中表达的意义。方法:选取河南科技大学第一附属医院病理科2008~2010年存档的子宫内膜组织标本84例,根据不同组织学类型分为:正常增殖期子宫内膜组(12例)、子宫内膜不典型增生组(17例)与子宫内膜腺癌组(55例)。应用免疫组化SP技术分别分析cIAP蛋白和Caspase-7蛋白在各组中的表达情况。分别分析cIAP蛋白和Caspase-7蛋白与55例子宫内膜腺癌患者的临床分期、病理分级、肌层浸润等临床生物学特性的关系。55例子宫内膜腺癌组患者均行外科分期手术,包括:I~II期37例,III~IV期18例;共有45例患者行淋巴清扫术,发现有淋巴结转移18例。分析cIAP蛋白和Caspase-7蛋白在55例子宫内膜腺癌中表达的相关性。所选组织标本患者均无其它系统恶性肿瘤,手术病人术前均未行放射和化学治疗,所有组织标本均由河南科技大学第一附属医院病理科固定(10%福尔马林液)、包埋(石蜡),由两位及以上资深病理专业医师阅片。本实验数据采用SPSS17.0统计学软件,运用χ2检验、Spearman相关分析进行分析,以P0.05为差异有统计学意义。结果:cIAP蛋白和Caspase-7蛋白阳性信号在子宫内膜上皮细胞中定位于细胞质,呈棕黄色颗粒。cIAP蛋白的阳性表达分别为:子宫内膜腺癌组(43/55,78.2%),子宫内膜不典型生组(5/17,29.4%),正常增殖期子宫内膜组(3/12,25.0%),三组比较差异具有统计学意义(P0.05)。cIAP的表达与55例子宫内膜腺癌患者的临床分期、病理分级、淋巴转移有关(P0.05),而与子宫肌层浸润及患者年龄无关(P0.05)。Caspase-7蛋白的阳性表达分别为:子宫内膜腺癌组(20/55,36.3%),子宫内膜不典型增生组(13/17,76.4%),正常增殖期子宫内膜组(10/12,83.3%),三组相比差异有统计学意义(P0.05),Caspase-7的表达与55例子宫内膜腺癌患者的临床分期、病理分级、淋巴转移有关(P0.05),而与子宫肌层浸润及患者年龄无关(P0.05)。采用Spearman相关分析,cIAP蛋白和Caspase-7蛋白在55例子宫内膜腺癌标本中表达呈负相关(rs=-0.607,P0.05)。结论:1.与正常增殖期子宫内膜组及子宫内膜不典型增生组相比较,凋亡抑制蛋白cIAP在子宫内膜腺癌组中高表达,凋亡蛋白Caspase-7在子宫内膜腺癌组中低表达,提示细胞凋亡紊乱可能参与了子宫内膜组织的恶性增殖转化。2.cIAP和Caspase-7在子宫内膜腺癌中的表达均与患者病理分级、临床分期及有无淋巴结转移相关,而与患者年龄和肿瘤侵犯子宫肌层深度无关,提示二者可能在子宫内膜腺癌的侵袭进展中发挥作用。3.cIAP和Caspase-7在子宫内膜腺癌组织中的表达呈负相关,提示二者在子宫内膜腺癌的发展过程中相互制约,两因子联合检测可能对评估子宫内膜腺癌的临床预后提供理论依据。
[Abstract]:Objective: To study the expression of apoptosis inhibitory protein cIAP and apoptotic protein Caspase-7 in different endometrium tissues, and analyze the relationship between the expression of two and the clinical biologic characteristics of endometrial adenocarcinoma clinical staging (FIGO 2009), pathological classification, lymph node metastasis, and the correlation between the two expressions, and explore the two in endometrium group. Methods: 84 cases of endometrial tissue in the pathology department of the First Affiliated Hospital of Henan University of Science and Technology were selected for 2008~2010 years. The expression of Caspase-7 was related to the clinical stage, pathological grade and lymphatic metastasis (P0.05) in 55 cases of endometrial adenocarcinoma, but it was not related to the infiltration of myometrium and the age of the patients (P0.05). The expression of cIAP protein and Caspase-7 protein was negative in the specimens of endometrial adenocarcinoma in 55 cases. Conclusion: (rs=-0.607, P0.05) conclusion: 1. compared with the normal proliferative endometrium group and the endometrial dysplasia group, the apoptosis inhibitory protein cIAP is highly expressed in the endometrial adenocarcinoma group, and the apoptotic protein Caspase-7 is low expression in the endometrial adenocarcinoma group, suggesting that the apoptosis disorder may be involved in the malignant proliferation of endometrium. The expression of.2.cIAP and Caspase-7 in endometrial adenocarcinoma is related to the pathological grading, clinical stage and lymph node metastasis, but not related to the age of the patients and the depth of the invasion of the uterine myometrium, suggesting that the two may play.3.cIAP and Caspase-7 in endometrial adenocarcinoma tissue in the progression of endometrial adenocarcinoma. The expression is negatively correlated, suggesting that the two are mutually restricted during the development of endometrial adenocarcinoma. The combined detection of two factors may provide a theoretical basis for the evaluation of the clinical prognosis of endometrial adenocarcinoma.
