AR及BMP-2在浸润性乳腺癌组织中的表达及临床病理意义

【摘要】:目的:探讨浸润性乳腺癌患者原发灶中骨形成蛋白(bone morphogenetic protein-2,BMP-2)及雄激素受体(androgen receptor,AR)的表达情况,并探讨其与临床病理特征的关系。方法:收集新疆医科大学附属肿瘤医院2009年1月至2014年12月经手术切除并经病理诊断的浸润性乳腺癌123例,采用免疫组织化学方法检测AR,BMP-2在浸润性乳腺癌患者原发灶中的表达情况,并分析其与临床病理特征的相关性。结果:本研究所选123例浸润性乳腺癌病例中,BMP-2阳性共计114例(92.7%),AR阳性共计98例(79.7%);在肿瘤大小分组中,肿瘤5cm病例中AR阳性率100%,2-5cm病例中AR阳性率90.7%,"f2cm病例中AR阳性率71.6%,三组表达有统计学差异(=35.168,P0.01);在不同组织学分级分组中,Ⅰ级AR阳性率80%,Ⅱ级AR阳性率88.9%,Ⅲ级AR阳性率59.5%,三组表达差异有统计学意义(=14.494,P=0.025);Ⅰ级BMP-2阳性率80%,Ⅱ级BMP-2阳性率97.5%,Ⅲ级BMP-2阳性率83.3%,表达差异有统计学意义(=13.853,P=0.031);ER阳性组AR阳性率87.4%,ER阴性的AR阳性率53.6%,两组表达差异有统计学意义(=16.868,P=0.001);PR阳性组AR表达率87.5%,PR阴性组AR表达率60%,两组表达差异有统计学意义(=12.822,P=0.005);AR阳性组BMP-2阳性率95.9%,AR阴性组BMP-2阳性率80%,两组表达差异有统计学意义(=8.909,P=0.031);在分子分型中的阳性率分别为,Luminal A型(80.0%)、Luminal B型(87.4%)、Her-2过表达型(52.6%)、Basal-like型(0),差异有统计学意义(=23.456,P=0.005)。本研究中发生骨转移病例85例,未发生骨转移病例38例,有腋窝淋巴结转移组AR阳性率88.4%,无腋窝淋巴结转移组68.5%,两组之间AR表达差异有统计学意义(=8.615,P=0.035);有神经脉管侵犯组的AR阳性表达率为96.3%,无神经脉管侵犯组阳性表达率75.0%,两组之间AR表达差异有统计学意义(=34.143,P0.01);而在年龄,临床分期,骨转移、Her-2阳性、阴性组,Ki-67阳性、阴性组中的AR表达差异无统计学意义(P0.05);在年龄、临床分期、肿瘤大小、神经脉管侵犯、淋巴结转移、骨转移及ER,PR,Her-2,Ki-67等分组中BMP-2表达差异无统计学意义(P0.05)。结论:在组织学分级中,Ⅱ级BMP-2阳性率、AR阳性率高于Ⅰ级、三级;在肿瘤大小分组中,AR阳性率随着肿瘤直径的增大而增高。在分子分型中,AR阳性率由高到低分别为Luminal B型、Luminal A型、Her-2过表达型、Basal-like型,BMP-2表达率由高到低分别为Basal-like型、Luminal B、Luminal A型、Her-2过表达型。在淋巴结转移组中,AR阳性率在有腋窝淋巴结转移组高于无腋窝淋巴结转移组;在神经脉管侵犯组中,AR阳性表达率在有神经脉管侵犯组高于无神经脉管侵犯组。AR阳性组BMP-2阳性率高于AR阴性组BMP-2阳性率,而且AR高表达的组织中BMP-2也呈高表达,AR低表达的组织中BMP-2也呈低表达。AR与ER、PR在乳腺癌组织中的表达具有一定的相关性,并在乳腺癌的浸润及转移过程中发挥重要作用;BMP-2无法独立为浸润性乳腺癌患者骨转移提供风险预警。
