E-cadherin,Ki-67在胃癌中的表达及其意义

【摘要】:目的:在我国所有恶性肿瘤中对于胃癌的发生及发展仍是当前的研究重点,本文通过免疫组化方法检测E-cadherin及Ki-67在胃癌组织中的表达来研究其与胃癌发生进展的相关性,以期进一步讨论胃癌的发展及转移的分子机制以及为胃癌的基因靶向治疗提供理论依据。方法:收集南昌大学第四附属医院自2011年1月至2013年12月行手术切除并行病理检查确诊胃癌标本62例,并按胃癌标本分为以下几组,性别组、年龄组、肿瘤直径组、肿瘤部位组、分化程度组、浸润深度组、淋巴结转移组、远处转移组、TNM分期组,利用免疫组化法(MaxVision)检测胃癌组织标本中的E-cadherin及Ki-67表达的阳性率及阴性率以及20例正常胃组织,并用SPSS22.0分析他们与各组的相关性并对实验数据进行统计学分析,以及E-cadherin和Ki-67的关系。结果:1、在62例胃癌组织中E-cadherin及Ki-67的阳性率分别为48.4%(30/62),61.3%(38/62)。2、E-cadherin在正常胃组织中的阳性率100%,高于胃癌组中的表达,差异具有统计学意义(P0.05)。E-cadherin在高分化及中分化胃癌组中的阳性率81.3%(13/16)高于低分化胃癌组中的阳性率37%(17/46),差异显著具有统计学意义(P0.05),此外E-cadherin在胃癌浸润深度中T1+T2组中的阳性率66.7%(16/24)高于T3+T4组中的阳性率36.8%(14/38),在淋巴结转移中无淋巴结转移组中的阳性率65.2%(15/23)高于有淋巴结转移组中的阳性率38.5%(15/39),在远处转移中无远处转移组中的阳性率59.6%(28/47)高于有远处转移组中的阳性率13.3%(2/15),在TNM分期中Ⅰ+Ⅱ期组中的阳性率65.6%(21/32)高于Ⅲ+Ⅳ组中的阳性率30.0%(9/30),以上P值均0.05;而E-cadherin与胃癌患者的性别组、年龄组及肿瘤直径组和肿瘤部位组无关(P0.05)。3、Ki-67在正常胃组织中的阳性率为5%,低于胃癌组,差异具有显著性(P0.05)。Ki-67在胃癌中的表达显示其在胃癌浸润深度中T1+T2组中的阳性率41.7%(10/24)低于T3+T4组中的阳性率73.7%(28/38),在淋巴结转移中有淋巴结转移组中的阳性率74.4%(29/39)高于无淋巴结转移组中的阳性率39.1%(9/23),在TNM分期中Ⅲ+Ⅳ期组中的阳性率80.0%(24/30)高于Ⅰ+Ⅱ期组中的阳性率43.8%(14/32),以上P值0.05;而Ki-67与胃癌患者的性别组、年龄组、肿瘤部位组、肿瘤直径组、分化程度组、远处转移组并未发现相关性(P0.05)。4、在胃癌组织标本中E-cadherin以及Ki-67的表达的阳性率,经过SPSS22.0的Spearman秩相关进行相关性分析,结果提示:E-cadherin与Ki-67的表达呈负相关,其中rs值为-0.291,而P值0.05。结论:1、E-cadherin在胃癌中的表达与胃癌的浸润深度、淋巴转移、远处转移、分化程度及TNM分期呈负相关,提示其在胃癌中表达下降容易致使胃癌细胞具有侵袭力,发生更多的淋巴转移及远处转移。2、Ki-67在胃癌中的阳性率表达提示Ki-67与胃癌浸润深度、淋巴结转移、TNM分期呈正相关,也表明了Ki-67在胃癌中的阳性表达与胃癌的恶性程度及转移密切相关。3、在胃癌中E-cadherin与Ki-67的表达呈负相关。
[Abstract]:Objective: in order to study the relationship between the expression of E-cadherin and Ki-67 in gastric cancer tissues by immunohistochemical method, the occurrence and development of gastric cancer are still the focus of the present study in all malignant tumors in China. In order to further discuss the development and metastasis of gastric cancer molecular mechanism and provide a theoretical basis for gene targeted therapy of gastric cancer. Methods: from January 2011 to December 2013, 62 cases of gastric cancer were collected from the fourth affiliated Hospital of Nanchang University. The specimens were divided into the following groups: sex group, age group, tumor diameter group. The positive and negative rates of E-cadherin and Ki-67 expression in gastric cancer tissues were detected by immunohistochemical (MaxVision) method, including tumor location group, differentiation group, depth of invasion group, lymph node metastasis group, distant metastasis group, and 20 normal gastric tissues. SPSS22.0 was used to analyze their correlation with each group, and the experimental data were statistically analyzed, as well as the relationship between E-cadherin and Ki-67. Results the positive rates of E-cadherin and Ki-67 in 62 cases of gastric cancer were 48.4% (30 / 62), 61.3% (38 / 62) .2the positive rate of E-cadherin in normal gastric tissue was 100%, which was higher than that in gastric cancer group. The positive rate of E-cadherin in well-differentiated and moderately differentiated gastric cancer group was 81.3% (13 / 16), which was significantly higher than that in poorly differentiated gastric carcinoma group (17 / 46) (P0.05). In addition, the positive rate of E-cadherin in T1 / T2 group was significantly higher than that in poorly differentiated gastric carcinoma group (P0.05). The positive rate of sex was 66.7% (16 / 24) higher than that of T3 T4 group (36.8%, 14 / 38), the positive rate of lymph node metastasis group was 65.2% (15 / 23) higher than that of lymph node metastasis group (38.5%, 1539%), the positive rate of distant metastasis group was 59.6% (28 / 47) higher than that of non-distant metastasis group (28 / 47).
