无功能性胰腺神经内分泌肿瘤的临床分析

【摘要】：目的:总结、探讨无功能性胰腺神经内分泌肿瘤的诊断和外科治疗特点,进一步加深对该病的认识。方法:回顾性分析2002年1月至2014年12月福建医科大学附属第一医院收治的16例无功能性胰腺神经内分泌肿瘤患者的临床资料,分析、总结其临床特点。结果:43.8%体检发现,出现临床症状中:50.0%腹部闷痛、25.0%腹部包块;92.3%初诊BMI正常或超重。检测AFP、CEA、CA125、CA199均阴性。肿瘤93.8%单发、6.2%多发(胰头1个、胰体1个),最大直径平均5.16cm,94.1%≥2.0cm、52.9%≥5.0cm;41.2%合并囊性变,23.5%合并钙化;部位:胰头47.1%、胰体35.3%、胰尾17.6%。B超、CT、MRI检出率均为100%;9例行B超检查,表现:77.8%界清,88.9%低回声;15例行CT检查,表现:62.5%界清,平扫93.8%低密度,增强86.7%明显强化,其中86.7%不均匀强化;4例行MRI检查,表现:平扫75.0%T1WI低信号、100%T2WI高信号,增强100%明显强化。CT扫描联合钙化表现、CT扫描联合囊性变表现、CT扫描联合钙化与囊性变表现、联合CT平扫低密度与增强明显强化表现、联合B超低回声与CT平扫低密度增强明显强化表现者可降低误诊率。13行手术切除,3例非手术治疗;15例获得随访,早期患者、术后患者预后较好。结论:无功能性胰腺神经内分泌肿瘤临床症状不典型,早期诊断困难,但影像学表现有特异之处;早期手术切除预后良好。
[Abstract]:Objective: to investigate the diagnosis and surgical treatment of nonfunctional neuroendocrine tumors of pancreas. Methods: the clinical data of 16 patients with nonfunctional pancreatic neuroendocrine tumors admitted to the first affiliated Hospital of Fujian Medical University from January 2002 to December 2014 were retrospectively analyzed and their clinical characteristics were summarized. Results: 43.8% of the clinical symptoms were found: 50.0% abdominal pain, 25.0% abdominal mass and 92.3% BMI were normal or overweight. AFP,CEA,CA125,CA199 was negative. 93.8% of the tumors were single, 6.2% had multiple tumors (1 head of pancreas and 1 body of pancreas), and the mean maximum diameter was 5.16 cm ~ (-1) 鈮，
