有营养风险的老年结直肠癌患者术前肠内营养的临床研究

【摘要】:目的:观察术前给予肠内营养支持后,对于有营养风险的老年结直肠癌患者在接受手术治疗后的营养状态、免疫功能和并发症的情况。方法:运用NRS2002营养风险筛查标准,选择出我院70例老年结直肠癌且合并有营养风险的患者,随机分为肠内营养组(EN组)36例,对照组34例。肠内营养组术前3天给予肠内营养支持,在接受手术治疗后第1、3、5、7天检测血浆总蛋白、前清蛋白、清蛋白、转铁蛋白、淋巴细胞总数、血浆D-乳酸(D-lactic acid,D-LAC)和二胺氧化酶(diamine oxidase,DAO),观察术中肠道清洁度、术后并发症。结果:在术后第1、3、5、7天的血浆总蛋白、前清蛋白、清蛋白、转铁蛋白及淋巴细胞总数的比较中,肠内营养组均明显高于对照组;D-LAC、DAO水平及腹腔感染、伤口感染等感染性并发症的发生率显著低于对照组,两组差异有统计学意义;两组在肠道清洁度、吻合口漏发生率上,差异无统计学意义(P0.05)。结论:老年结直肠癌合并营养风险时,术前给予肠内营养支持,可显著提高手术后的营养状态和免疫力,降低并发症。
[Abstract]:Objective: to observe the nutritional status, immune function and complications of elderly colorectal cancer patients with nutritional risk after preoperative enteral nutrition support. Methods: 70 elderly colorectal cancer patients with nutritional risk were randomly divided into EN group (36 cases) and control group (34 cases). The enteral nutrition group was given enteral nutrition support 3 days before operation. Plasma total protein, prealbumin, transferrin, lymphocyte count and plasma D-lactic acid, were measured on the 1st day after operation. D-LAC) and diamine oxidase (diamine oxidase,DAO) were used to observe the intestinal cleanliness and postoperative complications. Results: in the comparison of plasma total protein, prealbumin, transferrin and lymphocyte count on the 7th day after operation, enteral nutrition group was significantly higher than the control group. The levels of D-LACU Dao and the incidence of infectious complications, such as intraperitoneal infection and wound infection, were significantly lower than those in the control group, and the difference between the two groups was statistically significant. There was no significant difference in intestinal cleanliness and anastomotic leakage between the two groups (P0.05). Conclusion: in elderly patients with colorectal cancer associated with nutritional risk, preoperative enteral nutrition support can significantly improve the nutritional status and immunity after surgery, and reduce complications.
