外周血血小板与绝对淋巴细胞计数比值与外周T细胞淋巴瘤非特指型患者预后的关系

【摘要】：目的:分析治疗前外周血血小板与绝对淋巴细胞计数比值(plateletlymphocyte ratio,PLR)与外周T细胞淋巴瘤非特指型(peripheral T cell lymphoma,unspecified,PTCL-U)患者临床病理特征的相关性,并探讨其在预后中的意义。方法:对天津医科大学肿瘤医院初诊的163例PTCL-U患者的临床资料及生存情况进行回顾性分析。根据受试者工作特征(receiver operating characteristic,ROC)曲线的截断值将患者分为低PLR组(PLR170,80例)和高PLR组(PLR≥170,83例),分析治疗前PLR与PTCL-U患者预后的关系。结果:163例PTCL-U患者中,低PLR组患者的5年总生存(overall survival,OS)率和无进展生存(progression-free survival,PFS)率(76.3%和60.0%)均高于高PLR组(10.8%和8.4%,P值均0.05)。年龄、美国东部肿瘤协作组体力状态(Eastern Cooperative Oncology Group performance status,ECOG PS)评分、B组症状、血清乳酸脱氢酶(lactate dehydrogenase,LDH)水平、血小板及PLR与PTCL-U患者的OS明显相关(P值均0.01)。多因素分析结果显示,年龄、血清LDH水平、血小板及PLR是PTCL-U患者预后的独立影响因素(P值均0.05)。结论:治疗前外周血PLR与PTCL-U患者的预后明显相关,高水平PLR提示PTCL-U患者的预后不良。
