血清VEGF表达与肝癌TACE疗效关系的研究

【摘要】：目的:通过检测原发性肝癌患者经肝动脉化疗栓塞(TACE)治疗前后血清血管内皮生长因子(VEGF)水平,探讨原发性肝癌患者TACE治疗前后血清VEGF水平变化与疗效之间的关系。方法:选择2013年1月至2014年6月在我院介入治疗科接受TACE治疗的原发性肝癌患者40例为研究对象,采用酶联免疫吸附试验(ELISA)分别检测肝癌患者TACE治疗前1d及术后28d的血清VEGF、分析肝癌患者血清VEGF水平与患者临床基本资料、甲胎蛋白(AFP)及患者预后的关系。结果:40例肝癌患者TACE术前的血清VEGF水平为418.4±111.2pg/ml。HCC组患者TACE术前血清VEGF水平明显高于肝硬化组及健康对照组,差异均具有统计学意义(t值分别为4.839和10.895,P均0.05)。肝癌患者血清VEGF水平与患者临床基本资料均无明显相关(P均0.05)。肝癌患者TACE术后28d VEGF水平疗效较好组为424.6±103.2 pg/ml,较术前无明显差异,差异无统计学意义(t值=0.403,P0.05),疗效较差组术后28d VEGF水平为622.8±107.3 pg/ml,差异有统计学意义(t=2.098,P0.05)。肝癌患者术前VEGF水平与AFP、肝生化功能功指标无相关性(P均0.05)。将40例肝癌患者按TACE治疗疗效分为:疗效较好组26例与疗效较差组14例,疗效较差组患者术后28d血清VEGF水平高于疗效较好组,差异有统计学意义(P0.05),TACE治疗前后血清VEGF水平变化率与术后肿瘤病灶碘油存积情况呈显著相关性(r=-0.721,P0.05)。结论:肝癌患者血清VEGF水平高于肝硬化患者及健康人;血清VEGF水平与患者临床资料无关;TACE术前血清VEGF水平与AFP及GGT、LDH、AFU、ALP等生化指标无明显相关性;术后血清VEGF水平可以预测患者TACE治疗疗效。

Enzyme-linked immunosorbent assay (ELISA) was used to detect the relationship between serum VEGF level and clinical data, alpha-fetoprotein (AFP) and prognosis in patients with liver cancer one day before and 28 days after TACE treatment. Results: the serum VEGF level of 40 patients with liver cancer before TACE was 418.4 卤111.2pg/ml.HCC group, which was significantly higher than that of liver sclerosis group and healthy control group before TACE. The difference was statistically significant (t = 4.839 and 10.895, P < 0.05). There was no significant correlation between serum VEGF level and basic clinical data in patients with liver cancer.
