腹腔神经丛阻滞治疗不可切除胰腺癌相关性疼痛的荟萃分析

　　背景与目的:胰腺癌相关性疼痛的发生率高达50%-90%,单凭传统的药物治疗很难取得满意的镇痛效果。腹腔神经丛阻滞（CPB）虽被认为是一种有效的镇痛方法,但CPB联合药物对比传统的药物治疗胰腺癌相关性疼痛的临床研究却报告了相互矛盾的镇痛效果。本研究旨在评价CPB在胰腺癌相关性疼痛治疗中,尤其是初始止痛治疗中的地位和价值。方法:计算机检索Pubmed、Medline、Embase、Cochrane Library等数据库,搜集CPB联合药物对比传统的药物治疗胰腺癌相关性疼痛的随机对照试验,并比较两组VAS（视觉模拟法）疼痛评分,以及阿片类药物消耗量、不良反应、生活质量（QOL）和生存期。结果:包含548名不可切除的胰腺癌相关性疼痛患者的9项RCT组成了我们的荟萃分析。试验组288名患者接受CPB联合药物镇痛,对照组260名患者只接受传统的药物镇痛。合并分析结果显示:与基线相比,试验组在2周（WMD=-3.08,95%CI:-5.57 to-0.58）、4周（WMD=-3.67,95%CI:-5.88 to-1.45）、8周（W MD=-3.23,95%CI:-4.97 to-1.49）、1...
