【摘要】：《伤寒论》由张仲景所著,是现存最早的理法方药俱全的系统论述外感热病的临床医书,其,是最早提出辨证论治的医学著作。张仲景在继承《黄帝内经》对外感热病的认识的基础上,结合临床实践,极大地丰富了原有内容,创立了其独有的六经辨证体系,六经辨证归纳了疾病的寒热趋向、邪正盛衰,基本上概括了脏腑与十二经的病变,提示了外感病变化发展的规律,完善了外感热病的辨证论治体系,在中国医学史上具有无可比拟的重要地位,并启发了后世温病学三焦辨证的创立。目的:本课题旨在通过收集并阅读李培生教授发表的学术论文及著作,总结其学术思想,并通过收集整理《伤寒论》中与“发热”有关的条文及李培生教授医学著作中对于相关条文的解析,分析李培生教授对于相关条文的理解和对于外感热病的认识,总结归纳其对于治疗外感发热疾病的临床经验与学术思想。方法:本研究采用文献分析、归纳的研究方法,通过阅读文献总结李培生教授研究《伤寒论》的学术思想,并分析《伤寒论》中包含发热相关症状的条文与李培生教授对相关条文的分析,从辨证要点、病因病机、治法、方药、类似证等方面进行总结归纳,结合其他医家的理解,总结李培生教授对于治疗外感热病的独特思想。结论:通过研究,本文得出如下结论:太阳病为表证,以发热恶寒并见为特点,是外感病的初期阶段,本阶段正气奋起御外,当用汗法助正气祛邪外出,病轻者用桂枝汤类,病重者用麻黄汤类,若有表里同病者,则随其症治之;本病误下邪热内陷可导致痞证、脏结等,用陷胸汤泄热行水,用泻心汤除痞。阳明病为里实热证,发热以发热不恶寒、汗出、热势高为特点,其病煎灼津液,慎不可妄用汗下;热在气分、弥漫内外用白虎汤清热,有津伤气耗用白虎加人参汤;若邪热与燥矢互结腑气不通大便不下,用承气汤类通下腑实;中焦热与湿互结致胆汁疏泄失常而外溢,阳黄兼表用麻黄连翘赤豆汤、阳黄热重湿轻用栀子柏皮汤、阳黄兼里证腹满用茵陈蒿汤。少阳病发热以寒热往来为特点,兼见口苦、咽干,目眩,是邪居于半表半里,治之以小柴胡汤和解少阳;少阳病发热分为小柴胡汤证及少阳病变证,随证治之。太阴病多恶寒,若有脾阳虚兼表可发热,见恶寒重发热轻,不可峻发汗,当用桂枝汤缓发其汗;若脾阳虚下利较甚用表里双解之法而以温里为主,治之以桂枝加人参汤。少阴病发热多为阴虚发热,多见发热与心烦、口渴咽干并见,用清热育阴的黄连阿胶汤、猪芩汤等;阴虚兼太阳表实证见发热恶寒无汗、足冷、脉反沉,用麻黄附子细辛汤或麻黄附子甘草汤。厥阴病发热可分为寒热错杂与厥阴热利,若有寒热错杂证,治疗当寒温并施,方用乌梅丸清上温下、安蛔止利;上热下寒吐逆自利或咽喉不利、唾脓血,治以清上温下补虚;有利下粘腻脓血,腹痛,里急后重,肛门灼热,口渴,脉数有力者,以苦寒的白头翁汤治之。
