疏肝凉血汤联合心理干预治疗寻常型血热证银屑病的疗效观察

【摘要】：目的:通过临床调查研究,观察患者治疗前后皮损及心理学指标的变化,评价疏肝凉血汤联合心理干预治疗寻常型血热证银屑病的临床疗效及对患者生活质量的影响,为今后更深入的探索治疗银屑病的有效方法提供参考。方法:病例共90例,均来自山东中医药大学附属医院皮肤科,随机分为A、B、C三组,各30例,A组口服自拟疏肝凉血汤,外用龙珠软膏,并定期进行心理干预;B组口服疏肝凉血汤,外用龙珠软膏;C组口服消银胶囊,外用龙珠软膏,疗程为1个月。观察1月后三组患者的临床疗效,心理学指标及中医伴随症状,并进行统计分析。结果:1.临床疗效比较:A、B、C三组的治愈率分别为40%、40%、10%;总有效率分别为93.3%、90.0%、83.3%。A组优于B组、C组(P0.05),B组优于C组(P0.05)。疏肝凉血汤联合心理干预治疗临床疗效优于单纯应用汤剂治疗,汤剂优于中成药。2.心理学指标比较:A组SDS、SAS、SCL-90、DLQI评分均较前有显著降低,B组除焦虑外,余评分均低于治疗前,C组除偏执较前有所降低外,其他均无明显变化;A、B组比较,除强迫症状外均低于B组,A、C组比较,所有评分均低于C组(P0.05),B、C组比较,除焦虑及恐怖外均低于C组。疏肝凉血汤联合心理干预治疗,能有效缓解患者的负性情绪,提高患者的生活质量;中药疏肝凉血汤也能够在一定程度上缓解患者负面情绪,提高生活质量;消银胶囊在缓解患者不良情绪提高生活质量方面无明显作用。3.中医伴随症状:治疗后中医伴随症状阳性率较前降低,经统计学分析除苔黄、脉弦滑外差异均具有统计学意义(P0.05)。中药联合心理干预对改善中医伴随症状有临床意义。结论:疏肝凉血汤联合心理干预治疗能明显提高寻常型血热证银屑病临床疗效,并且能够改善患者心理状态,提高生活质量。
[Abstract]:Objective: to observe the changes of skin lesions and psychological indexes before and after treatment through clinical investigation. Objective: to evaluate the clinical efficacy of Shugan Liangxue decoction combined with psychological intervention in the treatment of psoriasis vulgaris and the effect on the quality of life of the patients. Methods: 90 cases of psoriasis, all from the dermatology department of affiliated hospital of Shandong University of traditional Chinese Medicine, were randomly divided into three groups, 30 cases each. Group A was treated with self-made decoction of soothing liver and cooling blood, external application of Longzhu ointment, and regular psychological intervention. Group B, oral Shugan Liangxue decoction, external application of Longzhu ointment; Group C was treated with Xiaoyin capsule and Longzhu ointment for one month. The clinical efficacy, psychological indexes and associated symptoms of TCM were observed in the three groups after January. Results: 1. Comparison of the clinical efficacy of the three groups: the cure rates of the three groups were 40 and 4010, respectively. The total effective rate was 93.3% and 93.3%, respectively. Group A was better than group B (P 0.05). Group B was superior to group C (P 0.05). The clinical curative effect of Shugan Liangxue decoction combined with psychological intervention was better than that of pure decoction and traditional Chinese medicine .2.The psychological index was compared with SDSSAS in group A. The scores of DLQI in SCL-90 group were significantly lower than those in group B except anxiety, and the scores in group C were lower than those in group C before treatment except that before treatment. All the scores in group A were lower than those in group C except obsessive-compulsive symptoms, and all the scores were lower than those in group C (P 0.05). Except anxiety and phobia, it was lower than group C. Shugan Liangxue decoction combined with psychological intervention therapy could effectively relieve the negative emotion of the patients and improve the quality of life of the patients. Traditional Chinese medicine Shugan Liangxue decoction can also alleviate the negative emotions of patients to a certain extent, improve the quality of life; Xiaoyin capsule has no obvious effect on alleviating the bad emotion and improving the quality of life. 3. TCM associated symptoms: after treatment, the positive rate of TCM associated symptoms is lower than before, after statistical analysis to remove yellow fur. The difference of pulse-chord slip was statistically significant (P0.05). Conclusion: Shugan Liangxue decoction combined with psychological intervention can improve the clinical efficacy of psoriasis vulgaris with blood heat syndrome. And can improve the psychological state of patients, improve the quality of life.
